SENIOR SERVICES DIVISION
POSITION: CASE MANAGER
Non-exempt/full-time
Summary: Under the supervision of the Program Director and Program Manager, the Case Manager is responsible for providing case management services which includes comprehensive assessment of senior clients, development of a care plan, coordination of services, and client follow-up. Responsibilities include in-home assessments, completing paper documentation and entry into software systems for the City of Los Angeles, maintaining contact with clients, monitoring progress and assessing needs for additional services. Writing accurate client assessment letters, emails, and other forms of communication. Provide members of the senior community and clients with referrals and follow-up to ensure services have been rendered.
Qualifications:
2 years of experience in the human services field and/or 2 or more years working with older adults and at-risk populations. Must be organized, computer literate, pass required evidence-based program certifications, able to communicate effectively with others and have excellent time management skills. In addition, should be aware of privacy laws and understand the need to maintain client confidentiality.
Compensation based on experience:
$20 – $24/ hr
