Make a difference while keeping the flexibility you need!
Provide physical therapy treatments under the direction of a licensed Physical Therapist
Help residents improve mobility, strength, balance, endurance, and overall function
Carry out individualized plans of care established by the PT
Monitor resident progress and communicate changes to the supervising therapist
Complete accurate and timely documentation using Net Health
Collaborate with therapy, nursing, and other members of the interdisciplinary team
Educate and encourage residents throughout their rehabilitation journey
Maintain compliance with facility, state, federal, and professional standards
Graduate of an accredited Physical Therapist Assistant program
Current New York PTA license or eligibility for NY licensure
SNF/LTC experience preferred
Strong communication and teamwork skills
Dependable, motivated, and ready to jump in when needed
A positive attitude and passion for helping residents reach their goals
