Our team delivers Concierge Primary Care, Urgent Care, Hormone Optimization, Weight Loss, IV Therapy, Patient Advocacy, and Advanced Diagnostic Testing directly to our clients’ homes throughout the Charleston area.
This is NOT traditional home health or hospice. We are not completing routine skilled nursing visits or managing long-term home health episodes.
Instead, our Registered Nurses serve as Clinical Concierges—becoming a trusted extension of both the patient and the medical provider.
Think of the role as an Executive Assistant for your client’s health.
Rather than simply completing visits, you’ll embed yourself into each client’s healthcare journey, helping them navigate the complexities of modern medicine. You’ll advocate on their behalf, educate them and their families, identify concerns before they become emergencies, and deliver treatment under the guidance of our Nurse Practitioners and Physicians.
Your assessment skills become the foundation that allows our providers to make confident clinical decisions from a distance. Whether you’re responding to an urgent illness, optimizing a patient’s long-term health, coordinating specialist care, or helping someone better understand their treatment plan, you’ll play an essential role in improving both outcomes and the patient experience.

What You’ll Do
As a Clinical Concierge RN, you’ll become the primary clinical advocate for your assigned clients while serving as the eyes, ears, and trusted extension of our provider team.
Beyond performing clinical assessments and treatments, you’ll help coordinate care, anticipate healthcare needs, educate patients and families, and ensure every client has a knowledgeable clinician walking beside them throughout their healthcare journey.
Responsibilities include:
- Comprehensive head-to-toe assessments and focused clinical evaluations
- Developing trusted, long-term relationships with patients and families
- Serving as the primary clinical liaison between patients, providers, specialists, caregivers, and family members
- Patient advocacy across the healthcare continuum—including emergency departments, hospitalizations, specialist referrals, and transitions of care
- Blood draws and specimen collection
- IV insertion, infusion therapy, injections, and medication management
- Point-of-care diagnostic testing and ECG acquisition
- Assisting with in-home Primary Care and Urgent Care visits
- Providing education regarding medications, disease processes, prevention, nutrition, and longevity strategies
- Identifying subtle clinical changes early and communicating recommendations to providers
- Coordinating follow-up care and helping patients remain engaged with their health goals
- Accurate documentation within our electronic medical record
No two days are alike.
One visit may involve treating an acute illness, while the next may include helping a patient optimize hormone therapy, performing advanced metabolic testing, attending a specialist appointment as their advocate, or coordinating care after a hospital discharge. Every day brings a different opportunity to improve someone’s health—and their confidence navigating the healthcare system.
Pay: $40.00 - $50.00 per hour
