This position is located at the SC Department of Behavioral Health and Developmental Disabilities (DBHDD) - Office of Mental Health, Tri-County Mental Health Center, 207 Perr Wiley Wy, Chesterfield, SC 29709
As the Intensive Community Treatment Counselor I, you will provide in-home rehabilitative services (RBBHS) under the supervision of the Intensive Community Treatment (ICT)Coordinator and will serve on average, 20-25 patients, with a maximum caseload not to exceed 35 patients at one time to patients of the Tri-County Community Mental Health Center - Marlboro County catchment area.
Responsibilities for the Counselor I:-
Provide quality in-home treatment services for patients who are receiving intensive community treatment (ICT). Render rehabilitative behavioral health services as prescribed by the plane of care.

Ensure all assigned caseloads and clinical records are in accordance with OMS Quality Assurance (QA) standards.

Complete and submit weekly case reviews to supervisor.

Make referrals to Clinical Care Coordination as needed based on patient's presenting needs outside of Tri-County Community Mental Health Center.

Utilize collaborative documentation to facilitate effective and efficient provision of therapeutic services.

Ability to interpret and apply laws, regulations, policies and procedures.
