Director of Patient Services (RN) – Concierge/Private Pay Home Care – $150K + Bonus – White Plains, NY - Office & Field Based

We are seeking a dynamic, sophisticated Director of Patient Services (DPS) to oversee our Licensed Home Care Services Agency (LHCSA) operations. Serving a distinguished geriatric population across Westchester County, this high-level leadership and compliance position focuses heavily on care coordination, caregiver management, and white-glove family relationship building rather than intensive bedside clinical tasks.
As the primary clinical leader, you will enjoy a high degree of daily flexibility and act as the nursing license holder for the organization, directly shaping the culture of quality and compliance that keeps our clients safely aging at home.
What You’ll Do: Concierge Care Oversight & Clinical Compliance
- Concierge Relationship Management: Interface directly with high-expectation, private-pay families to deliver responsive, high-touch communication, coordination, and clinical reassurance.
- Clinical Assessments & Care Planning: Complete comprehensive start-of-care physical and environmental assessments for approximately 55–65 clinical LHCSA clients, establishing detailed, personalized Plans of Care.
- Multidisciplinary Coordination: Prepare doctor's orders, manage six-month patient reassessments, and collaborate seamlessly with outside providers including hospice agencies, physical therapists, and CHHAs.
- Caregiver Leadership & Advocacy: Supervise, evaluate, and mentor our dedicated HHA and PCA staff. You will oversee orientations, review annual health paperwork, and design engaging in-service training sessions to elevate care quality.
- Regulatory Oversight: Ensure absolute compliance with New York State Department of Health (NYS DOH) regulations and OSHA standards, serving as the clinical backbone of the agency.
- Manageable Field Supervision: Maintain a highly balanced schedule with a manageable field caseload of 12–15 supervisory or assessment visits per week, fully supported by an established 8-person office operations team.
Schedule & Location
- Hours: Monday–Friday, 8:30 AM – 5:00 PM (with built-in flexibility to start a bit later as long as clinical needs are met).
- Accountability: 24-hour clinical accountability with minimal emergency phone triage (our care management team handles initial 24/7 routing; most calls are non-medical or directed straight to 911).
- In-Service Training: Two Saturdays per year are required on-site to lead essential caregiver in-service training sessions.
- Location: Blended office and field workflow based out of White Plains, NY (Free parking available!). A company car is available for use as needed.
Qualifications
Education & Licensure
- Registered Nurse (RN), licensed and currently registered to practice in New York State.
- BSN preferred.
- Valid Driver’s License and personal vehicle.
Experience
- 2–3 years of clinical nursing experience paired with clear charge nurse, supervisory, or leadership experience.
- A background in a LHCSA or home care environment is highly preferred, though we are open to training a highly motivated leader coming from another acute setting who is ready to master New York State regulations.

Compensation & Benefits
- Salary: Starting at $150,000 annually + Performance Bonus.
- Pay Frequency: Semi-monthly (15th and 30th of every month).
