Job Overview:
The Licensed Practical Nurse (LPN) provides compassionate, resident-centered nursing care in a long term care setting. The LPN is responsible for delivering skilled nursing services, administering medications and treatments, monitoring resident health status, and collaborating with the interdisciplinary care team to promote quality of life, safety, and overall well-being for residents.
Key Responsibilities:
- Provide direct nursing care to residents in accordance with individualized care plans, facility policies, and state/federal regulations.
- Administer medications and treatments safely and accurately while monitoring for effectiveness and adverse reactions.
- Perform nursing assessments, document resident conditions, and report changes in health status to the appropriate healthcare providers.
- Collaborate with physicians, registered nurses, certified nursing assistants, therapy staff, and families to support resident care goals.
- Assist with the development, implementation, and evaluation of resident care plans.
- Monitor vital signs, wounds, nutrition, hydration, and other aspects of resident health.
- Provide education and support to residents and families regarding care needs and treatment plans.
- Maintain accurate and timely clinical documentation according to regulatory and facility standards.
- Ensure compliance with infection prevention practices, safety procedures, and quality improvement initiatives.
- Supervise and support nursing assistants as needed to ensure safe and effective resident care.
- Promote dignity, respect, independence, and a positive environment for all residents.
Qualifications:
- Current Licensed Practical Nurse (LPN/LVN) license in the state of practice.
- Graduate of an accredited practical nursing program.
- Previous experience in long-term care, skilled nursing, rehabilitation, or geriatric nursing preferred.
- Current CPR certification required.
- Strong clinical assessment, communication, and organizational skills.
- Ability to work effectively as part of an interdisciplinary healthcare team.

- Knowledge of nursing practices, infection control standards, and long term care regulations.
Shift:
5x8 Hr. Days (8:30am - 5:00pm) (40 Hrs. per Week)

Pay Breakdown:
$1,480.00 Gross Weekly ($600.00 Taxable & $880.00 Tax-Free) ($37.00/Hr. Blended Rate)
