Patients First Home Care LLC is looking for a Physical Therapist to provide in-home visits to our patients.
Provide a comprehensive patient examination/evaluation. Complete Oasis assessments timely and accurately. Reassess patient every 30 days and make necessary revisions to the plan of care in conjunction with the physician and documents the patient's continual needs and progress.
Provides patient oversight and supervision of Physical Therapy Assistants. Supervision of Home Health Aides with updates to care plan as needed.
Participates in patient care conference, updates and assists other members of the care team. Participates in; in services, chart audits, quality improvement activities.
Requirements:
Current license to practice Physical Therapy in the State of Florida
Graduated from an accredited physical therapy educational program
One year of physical therapy experience. Preferably 1 year home health experience
A level II background screening is required for this position https://info.flclearinghouse.com
Current Driver's License with current auto insurance
CEU on Elderly Abuse, Human Trafficking and Alzheimer's/Dementia
TB test results
CPR Certification. Online Certifications are NOT accepted

Pay: $80.00 - $90.00 per hour
