RN or LPN Part Time or FT 11pm to 7am
Job Summary:

As a key member of our night shift, you will play a vital role in delivering high-quality patient care to our residents. Responsibilities:
Provide direct patient care to residents, including administering medications, assisting with daily living activities, and monitoring vital signs
Collaborate with the healthcare team to develop and implement individualized care plans
Conduct routine vital sign assessments, monitor and report changes in resident condition, and respond to emergencies
Maintain accurate and up-to-date records of patient care, medications, and treatments
Participate in incident reporting, quality improvement, and infection control initiatives
Provide education and support to residents and their families
Requirements:
Current licensure as an RN or LPN in the state of Illinois
High school diploma or equivalent required; associate's or bachelor's degree preferred
2 years of experience
Long term care or rehab experience preferred
Ability to work 11pm to 7am shifts, including rotating weekends and holidays
