Speech and Language Pathologist (SLP) – Part-Time (20 Hours/Week)
Position Type: Temporary to Hire (based on performance and business need)
Schedule: Monday–Friday, flexible schedule between 8:00 AM – 5:00 PM (20 hours per week)
Locations: Oakland, San Leandro, and Livermore, CA
Position Overview
We are seeking a compassionate and experienced Speech and Language Pathologist (SLP) to provide diagnostic and therapeutic speech-language services for older adults enrolled in a PACE (Program of All-Inclusive Care for the Elderly) program. This role focuses on evaluating and treating communication and swallowing disorders while collaborating with an interdisciplinary care team to help participants maintain their independence and quality of life.
This is a part-time, temp-to-hire opportunity supporting multiple PACE centers based on operational needs.
Key Responsibilities
- Evaluate participants with speech, language, communication, cognitive, and swallowing disorders.
- Develop individualized treatment plans and functional goals in collaboration with physicians and the interdisciplinary care team.
- Provide speech-language therapy in accordance with physician orders and participant care plans.
- Monitor participant progress, reassess as required, and maintain accurate clinical documentation.
- Educate participants, caregivers, and staff on therapy techniques and home exercise programs.
- Participate in interdisciplinary team meetings and contribute to participant care planning.
- Provide training and guidance to staff regarding participant speech therapy needs.
- Travel between assigned PACE centers and participant residences as needed.
Qualifications
- Master's degree in Speech-Language Pathology.
- Current California Speech-Language Pathologist license.
- Minimum of one (1) year of experience working with the elderly or frail adult population.
- Current CPR and First Aid certification.
- Valid California driver's license, reliable transportation, and current auto insurance.
- Ability to travel between multiple work sites and participant homes.
Physical Requirements
- Ability to sit for extended periods and perform computer-based documentation.
- Ability to occasionally stand, walk, bend, kneel, reach, push, pull, and lift up to 50 lbs.
- Ability to work in participant homes and clinical settings, including occasional exposure to biohazard materials and uneven walking surfaces.
Pay: $45.00 - $60.00 per hour
