<Job Overview>
We are seeking a motivated and energetic Registered Nurse (RN) to join our team. This role offers an exciting opportunity to deliver exceptional patient care directly in clients’ homes, utilizing your diverse clinical skills and expertise. As a vital member of our care team, you will assess patient needs, develop care plans, and provide hands-on nursing interventions to promote health and well-being, train CarePros, and supervise as well. Clients can be in MD, WV or VA.
<Duties>
Conduct comprehensive patient assessments, including vital signs, physical examinations, and health history evaluations.
Set up medications and review meds for compliance
Document all patient interactions accurately using EMR (Electronic Medical Records) system WellSky while maintaining HIPAA compliance.
Provide education on disease management, medication administration, nutrition, and health coaching tailored to individual needs.
Train CarePros on complex needs such as ostomy care, catheter care, etc.
Compliance with state regulations, to include state surveys
Travel to clients in any market served in WV, VA and MD.
Conduct CarePro training, supervisory visits which can be during evening and night hours, and ongoing staff training and assessment of CarePro skills
Other duties as assigned or needed to provide optimal care in the home
<Skills>
Proven experience with home care nursing, including long-term care, hospice & palliative medicine, post-acute settings.
Strong knowledge of medical terminology, anatomy, physiology, and diagnostic evaluation techniques.
Proficiency in EMR/EHR systems like WellSky or ability to learn
Ability to work independently while managing multiple patients in a home environment; excellent patient assessment skills.
Experience working with diverse populations including seniors, individuals with developmental disabilities or dementia care needs.

<Education & Experience>
Ability to establish and maintain good working relationship with clients, families, coworkers and CarePros
Ability to work with various agencies, referral sources and the general public
Registered Nurse license in VA, MD or WV or multi-state compact
Two years of nursing experience, in home health or home care will be a bonus
Driver's license, auto insurance required, clean driving record for use of company vehicles
Computer proficiency required
<Physical Qualifications>
Ability to transport self throughout service area
Ability to walk to and from patient's care setting to include stairs outside and inside the home or residential setting
Able to lift and move patients. Must lift 50lbs.
Ability to communicate clearly in English by writing, reading and speaking
Ability to withstand odors to include pet odors
Ability to be in home with animals such as cats, dogs, birds
Ability to use a computer, cell phone and durable medical equipment
Ability to sit, stand, walk in a variety of settings and locations
Job Type: Full-time
Pay: $29.00 - $39.00 per hour
