As we continue to grow, we are seeking a highly motivated, personable, and results-driven Outreach Coordinator to join our team! This rewarding role is ideal for someone who enjoys connecting with people, building meaningful relationships, and helping families access valuable services and support.
Position Overview:
The Outreach Coordinator will build and maintain relationships with families and Support Coordinators, assist families in accessing the services and supports provided through TeamCare, support marketing and outreach initiatives, guide families through the DDD application process, and serve as a resource for families seeking information, support, and services.
The ideal candidate will possess strong communication, organizational, and interpersonal skills, as well as exceptional attention to detail. Bilingual proficiency in Spanish is required. The candidate should also be comfortable traveling as needed to support the responsibilities of the position. Schedule

Qualifications
- Previous administrative experience required.
- Fluent in both English and Spanish.
- Existing professional relationships with New Jersey DDD Supports Coordinators
- Proven experience in community outreach, marketing, referral development, community relations, or a related field.
- Excellent communication and organizational skills.
- Strong attention to detail.
- Ability to build and maintain professional relationships.

As an Outreach Coordinator, you will
- Meet regularly with company leadership to support outreach initiatives.
- Assist families with the application process for New Jersey's Division of Developmental Disabilities (DDD) services.
- Conduct ongoing follow-up communications through phone calls, emails, and other channels.
- Support marketing and outreach efforts by attending community fairs, networking events, and professional meetings.
- Build and maintain relationships with Support Coordinators and community partners.
- Assist clients with transitioning from respite services to adult programs.
- Serve as a resource and advocate for families seeking support and services.
