Position Summary
The PRN Physical Therapist is responsible for evaluating residents, developing individualized treatment plans, and providing physical therapy services to residents in a long-term care and skilled nursing environment. The Physical Therapist works collaboratively with interdisciplinary team members to promote resident mobility, safety, independence, and overall quality of life while maintaining compliance with federal, state, and facility regulations.
Essential Job Functions
- Evaluate residents upon physician referral and develop individualized treatment plans based on clinical findings and functional goals.
- Provide skilled physical therapy interventions in accordance with physician orders and established plans of care.
- Assess resident progress and modify treatment plans as appropriate.
- Complete all required documentation accurately and within established timeframes, including evaluations, progress notes, recertifications, and discharge summaries.
- Collaborate with physicians, nursing staff, occupational therapists, speech therapists, social services, and other interdisciplinary team members regarding resident care and discharge planning.
- Educate residents, families, and caregivers regarding treatment plans, mobility techniques, safety precautions, and home exercise programs.
- Participate in care conferences and interdisciplinary meetings as requested.
- Ensure compliance with Medicare, Medicaid, insurance guidelines, and facility policies and procedures.
- Maintain infection control, safety, and confidentiality standards in accordance with HIPAA and facility requirements.
- Assist in maintaining therapy equipment and therapy treatment areas in a clean and safe condition.
- Demonstrate flexibility and availability to work PRN/as-needed shifts, including weekends or holidays as required by facility needs.
- Perform other duties as assigned within the scope of practice.
Qualifications
- Graduate of an accredited Physical Therapy program.
- Current and active Physical Therapist license in the applicable state.
- Current CPR/BLS certification required.
- Previous experience in skilled nursing, long-term care, rehabilitation, or geriatric settings preferred.
- Knowledge of rehabilitation techniques, therapeutic exercise programs, and mobility training.
- Strong communication, documentation, and interpersonal skills.
- Ability to work independently and collaboratively within an interdisciplinary team.
Physical Requirements
- Ability to stand, walk, bend, stoop, kneel, and assist residents throughout the workday.
- Ability to lift, transfer, and support residents with or without reasonable accommodation.
- Ability to push/pull therapy equipment and mobility devices.
This is a PRN/as-needed position. Hours and schedules may vary based on facility census and operational needs.
Pay: From $58.00 per hour
