Job Summary
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team. Responsibilities
- Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using electronic health record (EHR) systems such as Epic or Cerner.
- Administer medications and injections with precision, including IV infusions, ensuring adherence to HIPAA regulations and safety protocols.
- Manage complex care needs such as Alzheimer’s care, dementia care, memory care, hospice care, and acute pain management in various settings like nursing homes, hospitals, or home & community care environments.
- Assist with specialized procedures including catheterization, tube feeding, venipuncture, sterile processing, and wound suturing while maintaining aseptic techniques.
- Collaborate with multidisciplinary teams for case management, discharge planning, and patient education to optimize health outcomes.
- Conduct patient triage in urgent and emergency care settings; prioritize cases based on acuity levels such as Level I Trauma Center or ICU experience.
- Document all patient interactions accurately in medical records using EMR/EHR systems like Athenahealth or eClinicalWorks; ensure compliance with HIPAA standards.
- Support infection control practices and participate in quality improvement initiatives aligned with NCQA standards.
Requirements
- Valid state licensure as a Licensed Practical Nurse (LPN) with current certification to practice in healthcare settings.
- Proven experience working with diverse populations including seniors, individuals with developmental disabilities, pediatric patients, neonatal ventilator management or neonatal care experience is highly preferred.
- Strong knowledge of medical terminology, physiology, anatomy, ICD coding (ICD-9/ICD-10), CPT coding, and diagnostic evaluation procedures.
- Hands-on experience with EMR/EHR systems such as Epic, Cerner or Athenahealth; familiarity with medical imaging documentation is a plus.
- Ability to perform physical examinations, vital signs measurement, specimen collection/processing, and basic life support (BLS).
- Excellent communication skills for effective patient assessment and health coaching; capable of working independently or as part of a team in fast-paced environments like emergency rooms or urgent care clinics.
- Demonstrated proficiency in infection control practices and sterile technique; experience with airway management or ventilator support is advantageous.
- Flexibility to work across multiple settings including inpatient units such as ICU or PICU; outpatient clinics; home & community care programs; or assisted living facilities.
