Overview
Join our dynamic healthcare team as a dedicated Physical Therapist, where you will play a vital role to provide skilled therapy services to patients in their homes. The Physical Therapist will evaluate patients, develop individualized treatment plans, and implement evidence-based interventions to improve mobility, strength, balance, safety, and overall functional independence. This role focuses on helping patients remain safely in their homes while maximizing quality of life and reducing hospital readmissions.
Duties
- Conduct comprehensive assessments of patients’ physical conditions, including mobility, strength, and functional limitations.
- Perform comprehensive physical therapy evaluations in patients' homes.
- Develop and implement tailored care plans that incorporate evidence-based physiotherapy techniques to meet individual recovery goals.
- Deliver hands-on therapy sessions utilizing modalities such as trigger point therapy, manual therapy, and exercise prescription.
- Monitor patient progress regularly, adjusting treatment strategies to optimize outcomes.
- Collaborate with multidisciplinary teams to ensure cohesive patient care across inpatient, outpatient, hospital, home health, and clinic settings.
- Maintain accurate documentation of treatments, progress notes, and care plans using electronic medical record (EMR) systems while adhering to medical terminology standards.
Experience
- Proven experience in acute care hospital environments with a strong understanding of physiology and anatomy knowledge.
- Hands-on experience in outpatient clinics or home & community care settings providing patient-centered physiotherapy services.
- Knowledge of pediatrics, geriatrics, orthopedics, and geriatrics to address diverse patient populations effectively.
- Skilled in utilizing EMR systems for documentation and treatment planning.
- Demonstrated ability to work independently and as part of a multidisciplinary healthcare team.
- Additional certifications in physiotherapy techniques such as trigger point therapy or orthopedics are highly desirable. Pay: $65.00 - $75.00 per hour
