Job Overview
We are seeking a dedicated Licensed Clinical Social Worker (LCSW) to join our compassionate team. In this vital role, you will deliver comprehensive guests' relations, nursing supporting services: including social service, medical records, Guest's activities, etc.
Responsibilities
- Conduct thorough intake assessments and assist DON on guest's care plan;
- Support new admission process;
- Provide guests and family counseling and support;
- Document all guests' interactions accurately in guest's charts;
- Assist with discharge plan;
- Support residents’ social needs through management, community resource linkage, and family engagement efforts;
- Help guests and their family file Long-Term care insurance monthly submissions;
- Responsible for medical supplies and equipment orders;
- Organize facility social activity calendar and work with activity staff.;
- Work closely with hospice programs;
- Medical Records;
- Assist with podiatry and dental referrals;
- Guest Voting registration
- Assist with guest room settings.
Requirements
- Valid licensure as a Licensed Clinical Social Worker (LCSW) in the state of practice
- Proven experience in psychotherapy, mental health counseling, and behavioral health interventions
- Strong knowledge of Long-Term Care
- Experience working with seniors who have mental cognitive issues; such as dementia and Alzheimer’s disease;
- Familiarity with medical documentation standards including HIPAA compliance;
- Skills in motivational, organizational;
- Experience with State Survey will be helpful.

Pay: From $35.00 per hour
