Overview
MGS Home Health is seeking a per diem Field Registered Nurse (RN) in Oxnard. As a Field RN, you will play a crucial role in delivering high-quality healthcare services to patients in their homes. This position involves conducting patient assessments, providing skilled nursing care, and managing patient cases, all while ensuring compliance with health regulations and industry standards. The ideal candidate will have at least 1 year of experience in Home Health and be proficient in providing comprehensive care in the home environment.
Responsibilities
- Conduct thorough patient assessments to evaluate patients' health status, needs, and care requirements.
- Develop and implement individualized care plans in collaboration with physicians and healthcare teams.
- Perform skilled nursing procedures such as wound care, medication management, and patient education.
- Provide patient education on self-care techniques, medication management, and lifestyle changes.
- Coordinate care and maintain communication with patients, family members, and healthcare providers to ensure continuity of care.
- Monitor patient progress and adjust care plans as needed to ensure optimal health outcomes.
- Administer medications and treatments as prescribed, including IV therapy and injections.
- Document all patient interactions, assessments, and treatments accurately in compliance with home health regulations and organizational standards.
- Ensure infection control measures are in place and maintain a safe and sterile environment for both patients and staff.
- Lift and assist patients as necessary, ensuring their safety and comfort.
Skills
- At least 1 year of experience in Home Health nursing is required.
- Proficient in conducting SOC assessments and developing home health care plans.
- Skilled in wound care, IV therapy, medication management, and other home health nursing procedures.
- Strong case management abilities with excellent coordination and communication skills.
- Familiarity with home health documentation systems and legal requirements.
- Ability to educate and empower patients and their families on managing health conditions and following care plans.
- A valid California RN license and any necessary certifications are required.
Compensation: $60-$150
This is a 1099 position with payment on a per-visit basis.
Rate:
SOC - $150
ROC - $120
DC - $100
RECERT - $120
Follow-up Visit - $60
