This is a unique opportunity to make a lasting impact by supporting individuals with acquired brain injuries in a community-residential setting. As a CSC, you’ll play a vital role in helping our clients live more independently, reach personal milestones, and re-engage with their communities

Senior Supervisor / Supervisor
Schedule:
Per Diem / Full-Time / Part-Time – Up to 40 hours per week
Weekends and Holidays required
Full-time, part-time and per diem shifts available Sunday through Saturday:

8 a.m. - 4 p.m. (21 years old preferred)

8am - 4pm is weekends only (Saturday-Sunday) (21 years old preferred)
-
4 p.m. - Midnight (21 years old preferred)
-
12 a.m. - 8 a.m. ( May be 18 years old for this position)
Qualifications & Experience:
-
Minimum of High School Diploma or GED
-
At least 6 months of experience working or volunteering with individuals with disabilities (experience with acquired brain injuries a plus)

Bachelor’s degree is a plus

Valid driver’s license and an acceptable 5-year driving record (must be maintained)

Basic computer skills a plus; willingness to learn required

Ability to read, write, communicate effectively, and follow oral/written instructions

Physical ability to stand, walk, push/pull, and lift for most of the shift
Job Summary:
The Community Service Coordinator supports individuals with acquired brain injuries in a 24-hour community-based rehabilitation program. The role involves direct training, supervision, and transportation for clients, assisting with daily living activities, and promoting safety, independence, and overall quality of life in alignment with individualized goals. All activities are viewed as therapeutic interactions.
Essential Duties & Responsibilities:

Ensure timely and accurate delivery of client medications

Assist/supervise clients with daily living tasks (e.g., cooking, cleaning, hygiene)

Read and respond to work email at least twice per shift

Implement and follow individualized Plans of Care developed by the treatment team

Complete all required documentation (e.g., medication logs, goal sheets, behavior checklists)

Attend scheduled staff meetings, trainings, and in-service programs

Maintain professional appearance, communication, and behavior (per Employee Handbook)

Support team members and assist with client health, safety, and welfare as needed

Complete housekeeping and safety-related tasks (e.g., fire drills, visual checks)

Participate in community outings and recreational activities with clients (e.g., concerts, movies, shopping)

Uphold a positive, “can-do” attitude in all interactions

Cooperate with inspections and surveys conducted by State Licensing or Accreditation bodies

Accept additional responsibilities as assigned
Working Conditions:

Variable shifts including days, evenings, nights, weekends, holidays, and potential overtime

Work settings may include treatment areas, residential homes, or the broader community

All positions require staff to remain awake and alert throughout the entire shift

Emergency and unscheduled duties may be delegated by supervisory staff

Must work well as part of a team and demonstrate a strong commitment to collaboration across departments
