Licensed Vocational/Practical Nurse (LVN/LPN) – Home Health
Salary: From $40.00 per hour
Schedule: Monday to Friday | Day Shift | On-Call Availability

Are you passionate about providing high-quality care to patients in the comfort of their homes? Join our dedicated team at Millennium Home Health as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN). You’ll play an essential role in supporting patient health and well-being under the supervision of a Registered Nurse.
Key Responsibilities (include but are not limited to):
- Deliver direct, hands-on patient care in the home setting.
- Implement the plan of care initiated by the RN or provider.
- Ensure accurate and timely documentation in line with the plan of care.
- Educate patients and families/caregivers with information relevant to diagnoses and care plans.
- Coordinate with other members of the healthcare team and report needs to the RN or Clinical Supervisor.
- Use equipment and medical supplies effectively and efficiently.
- Participate in professional growth and development activities.
- Perform other duties as assigned by the supervising RN.
- Maintain patient confidentiality and adhere to HIPAA regulations.
- Supervise medical assistants when needed.
- Maintain a safe, clean working environment and follow infection-control protocols.

Qualifications:
- Current and active LVN or LPN license in the state of California (Required)
- Minimum 1 year of nursing experience (Home health or medical/surgical experience preferred)
- Current CPR certification (Required)
- Strong verbal and written communication skills
- Highly organized with attention to detail
- Must be a licensed driver with a reliable, insured vehicle that meets state and company requirements
- Willingness and ability to travel approximately 30–50 miles from home for patient visits
- Complies with accepted nursing standards and ethical practices
