In this vital role, you will work closely with licensed physical therapist/s to deliver exceptional patient care across diverse settings, including assisted living facilities/outpatient clinics, home environments (mobile outpatient therapy). *We have opportunity for both mobile outpatient therapy in the home as well as in assisted living facilities locally (on-site outpatient rehab program) in Denton county as well as Collin county
Responsibilities
- Assist licensed physical therapists in developing and implementing individualized care plans tailored to each patient’s needs.
- Provide hands-on therapies such as trigger point therapy, therapeutic exercises, manual techniques, and modalities to improve mobility and reduce pain.
- Monitor patient progress, document treatment sessions accurately, and communicate updates to the healthcare team.
- Support discharge planning by preparing patients for transition to home or other care settings through education and follow-up instructions.
- Educate patients and caregivers on physiotherapy techniques, safety measures, and injury prevention strategies.
- Maintain a clean, safe, and organized treatment environment in compliance with healthcare regulations and facility policies.
- Participate in interdisciplinary team meetings to coordinate care for patients across inpatient, outpatient, geriatrics, pediatrics, orthopedics, and other specialties.
Qualifications
- Valid certification or licensure as a Physical Therapist Assistant (PTA) in accordance with state regulations.
- Proven experience working in skilled nursing facilities, outpatient clinics, home health settings, or acute care environments.
- Strong knowledge of anatomy, physiology, medical terminology, and physiotherapy techniques.
- Skilled in providing specialized treatments such as trigger point therapy and managing care plans for diverse patient populations including geriatrics and pediatrics.
- Familiarity with inpatient and outpatient rehabilitation processes, discharge planning procedures, and orthopedic or sports-related therapy.
- Excellent communication skills to effectively collaborate with multidisciplinary teams and educate patients of all ages.
