We are currently looking for a full-time Licensed Practical Nurse (LPN) for home care department.
Responsibilities Include:
- Implement plan of care providing therapeutic nursing care and education to attain maximum patient and family independence and participation.
- Evaluate patient progress towards goals and confer with Case Manager regarding any potential changes in care prior to implementation.
- Assess goal achievement and service benefit to patients/families and consult with Case Manager regarding findings.
- Demonstrate flexibility in caring for patients with diverse physical and psychological needs.
- Record and document patient care services.
Qualifications:
- Current LPN licensure in MA.
- 1 - 2 years acute care experience required. Home care experience preferred.
- CPR certification.
- Valid driver's license and available transportation.
- Strong assessment and communication skills.
- Ability to build rapport with patients.
- Ability to handle physical workload,
