Title: Behavioral Case Manager II
Location: Los Angeles, Ca 90040
Schedule: 8 – 5 p.m., M-F, 4 days onsite, 1 remote
Hourly Rate: $24-26 an hour
Assignment Length: 12/30/26
Job Summary: The Case Manager II, Behavioral Health, advances the department’s mission by providing experienced referral management and case coordination for patients with behavioral health needs. The Case Manager manages and tracks referrals, coordinates communication between providers, and ensures timely linkage to care. The Case Manager II also documents all referral activities, assists with more complex patient navigation, and supports the team in addressing barriers to care.
Experience Required:
Associate’s degree in Social Services, Psychology, Human Services, Counseling, or related field required. Bachelor’s degree preferred. Minimum of 2 years of experience in healthcare, behavioral health, or social services, assisting patients with case management, referral coordination, and linkage to care, required. Bilingual English/Spanish or English and another predominant language of the population served preferred.
Experience Preferred:
Excellent communication and interpersonal skills for working with patients, families, and healthcare providers. Strong organizational and time management abilities, with capacity to prioritize and follow through on referrals. Experience navigating healthcare and social service systems and community resources. Ability to manage competing deadlines in a fast-paced environment. Proficient in documenting in Electronic Health Records (EHR) systems. Commitment to upholding patient confidentiality in accordance with program and HIPA.
