Job Purpose:
- The Licensed Practical Nurse at Crescent Community Health Center is responsible for providing high-quality, patient-centered healthcare services within the community. This role involves collaborating with healthcare teams to deliver compassionate care, ensuring patient comfort, and promoting overall health and wellness.
Key Responsibilities:
- Administer medications and treatments as prescribed by physicians and in accordance with nursing standards and protocols.
- Monitor patient health by conducting regular assessments, recording vital signs, and observing any changes in condition.
- Assist in the development and implementation of individualized patient care plans, ensuring they meet the specific needs of each patient.
- Provide education and support to patients and their families regarding health maintenance, disease prevention, and treatment options.
- Collaborate with physicians, registered nurses, and other healthcare professionals to ensure cohesive and comprehensive patient care.
- Maintain accurate and up-to-date patient records, documenting all care provided and any changes in patient condition.
- Participate in quality improvement initiatives to enhance patient care and operational efficiency within the health center.
- Ensure compliance with all healthcare regulations, policies, and procedures to uphold the highest standards of practice.
- Assist in maintaining a clean, safe, and organized healthcare environment for patients and staff.
- Engage in continuous professional development to stay current with nursing practices and healthcare advancements.
Required Education:
- Completion of an accredited Licensed Practical Nurse (LPN) program.
- Current LPN license in the state of practice.
Required Experience:
- Minimum of 1-2 years of experience working as a Licensed Practical Nurse in a healthcare setting.
- Experience in providing direct patient care and managing patient records.
Required Skills and Abilities:
- Strong understanding of nursing practices and procedures.
- Excellent communication skills to interact effectively with patients, families, and healthcare team members.
- Ability to assess patient needs and respond appropriately.
- Proficiency in using electronic health records and medical software.
