The Social Worker/Discharge Planner plays a critical role in coordinating safe and effective transitions of care for hospitalized patients. This position works closely with physicians, nurses, patients, families, and community resources to develop discharge plans that support recovery, reduce readmissions, and ensure continuity of care.

Key Responsibilities:

Conduct psychosocial assessments to identify patient needs, barriers to discharge, and available support systems

Develop and coordinate individualized discharge plans in collaboration with physicians, nurses, and care teams
-
Facilitate referrals to post-acute services including home health, skilled nursing facilities, rehabilitation centers, hospice, and community resources

Provide patient and family education regarding discharge plans and available support services
-
Assist patients and families with advance directives, end-of-life planning, and community resource connections

Collaborate with interdisciplinary teams to support length-of-stay management and care coordination
-
Coordinate transportation and post-discharge care arrangements as needed
-
Document discharge planning activities and patient interactions in the electronic health record

Monitor patient progress toward discharge goals and adjust plans as necessary
-
Support hospital initiatives focused on readmission reduction and quality improvement

Maintain compliance with CMS Conditions of Participation and regulatory standards related to discharge planning

Perform other duties as assigned
Qualifications
Education: Bachelor’s degree in Social Work (BSW) required. Master’s degree in Social Work (MSW) preferred.
Licensure/Certifications: Licensed Social Worker (LSW or LMSW) in the State of Mississippi required.
Experience: One (1) or more years of healthcare social work or discharge planning experience preferred. Experience in acute care hospital settings strongly preferred. Knowledge of post-acute care services, insurance requirements, and community resources.
