About the Role
We are seeking a highly skilled Psychiatric Mental Health Nurse Practitioner (PMHNP) specializing in Substance Abuse and Mental Health to join our multidisciplinary healthcare team. This role involves providing comprehensive psychiatric assessments, medication management, and therapeutic interventions for patients across diverse settings. The ideal candidate will possess extensive experience in behavioral health, trauma centers (hospitals, rehabs, detox,outpatient), ensuring high-quality patient care within a collaborative and patient-centered approach.
Proficiency with EMR/EHR systems is a plus.
Along with a strong foundation in psychopharmacology, patient assessment, and medical documentation.
This position offers an opportunity to work with a wide range of patient populations, including those with complex mental health needs, developmental disabilities, and co-occurring medical conditions.
Responsibilities
- Conduct thorough psychiatric evaluations and assessments utilizing your advanced clinical skills in mental health diagnostics.
- Develop individualized treatment plans incorporating psychopharmacology, therapy, and case management strategies.
- Manage medication administration, including injections and IV infusions, ensuring adherence to best practices and safety protocols.
- Collaborate with interdisciplinary teams across inpatient, outpatient, emergency medicine, hospice & palliative care, and post-acute settings to coordinate comprehensive patient care.
- Utilize EMR/EHR systems for accurate medical documentation and record keeping.
- Provide telehealth services leveraging tele-medicine platforms to expand access to behavioral health care.
- Participate in discharge planning, utilization review, and case management activities to optimize patient outcomes.
- Maintain compliance with Joint Commission and OASAS regulations and infection control standards while working with sensitive patient information.
Skills
- Extensive experience in behavioral health with proficiency in psychopharmacology and medication management.
- Proficient in EMR/EHR systems (No coding experience needed)
- Ability to perform physical examinations, vital signs assessment, psychoanalysis
- Experience working with diverse populations, such as individuals with addiction and mental health disorders
- Excellent communication skills for teaching hospital environments or outpatient clinics; adept at health coaching and discharge planning.
