Hmong Community Health Worker (CHW)
Organization: Healthy Opportunities for Latin Americans (HOLA) Inc.
Location: Central Wisconsin (Marathon, Portage, Lincoln, Langlade, Shawano, Wood, Clark, and Taylor Counties)
Reports to: Hmong Team Coordinator
Part-Time Position – 25 Hours/Week Includes in-person events, weekend hours, and some remote work

HOLA Purpose and Values: HOLA, Inc. focuses on empowering families through bilingual case management in Spanish, Portuguese and Hmong, healthcare navigation, food pantry support, cultural programming, and community engagement. Through it’s H2N (Hmong and Hispanic Communication Network) program it advances health equity for Hmong, Hispanic, and other vulnerable communities.
CHWs must demonstrate respect, empathy, patience, courtesy, and professionalism. Flexibility and adaptability in a rapidly changing environment are essential. Team members are expected to support one another and remain open to continuous learning.
Unique Role of Community Health Workers (CHWs): CHWs serve as trusted liaisons between families and service providers. They understand the social determinants of health and help clients overcome barriers to healthcare, social services, and community engagement. By combining H2N’s health equity framework with HOLA’s bilingual, culturally rooted services, CHWs ensure that Hispanic communities in Central Wisconsin receive both practical support and culturally competent care.
Specific Job Responsibilities:
● Assist clients with social needs (“case management”). Case management typically involves:
o Assessing Hmong client needs and connect them to necessary services and resources
o Acting as an advocate for clients with health service delivery agencies and personnel.
o Utilize client management Software to document and manage client cases properly
● Lead community-based blood pressure outreach/education and screenings in a variety of locations such as farms, churches, community centers, and existing community events.
● Improve health literacy, foster connections to primary care and case managers who can help address social determinants of health that might impede lifestyle changes and medical treatment.
● Staff community outreach events (weekday, evenings, and weekends)
● Engage with members of the community at clinics and events, disseminating materials.
● Attend and participate in weekly H2N team meetings and All-Staff Team meetings.
● Maintain knowledge of H2N and partner organizations’ available services and upcoming events for awareness outreach.
● Occasional travel to surrounding central Wisconsin areas for events.
o Marathon, Wood, Portage, Langlade, Clark, Taylor, Shawano, Lincoln
● Assist with project activities that advance health equity.
● Enhancing the organization’s reputation by being accountable and professional when engaging in collaboration with other partners
● If trained, facilitate Healthy Living with Diabetes, LENA Start, Stop the Bleed, and health screenings (Blood Pressure and Vision) or other education classes.
● Help translate documents and informational brochures and handouts to Hmong
● Attend networking events to learn about community resources and meet stakeholders
● Farmer’s Market Ambassador
o Attend weekly check-in meetings with Wood County Health Department Supervisor
o Occasionally go to Farmer’s Markets to check in Hmong sellers
o Host workshops to educate Hmong sellers on current trends or farmer’s market trends
o Attend monthly/regular ideation and collaborating meetings with Regional Farmers Market Coordinator (RFMC)
o Attend Central WI Farmers Market Collaborative meetings (occurring 6x/year - including an in-person meeting at Food + Farm Exploration Center in December)
o Be a resource/point of contact for any small-scale growers/farmer vendors that are needing assistance with payment technologies (token systems)
o Be a resource/point of contact for any new Hmong speaking farmers market vendors
● Other job-related tasks as assigned
Minimum Requirements
- Age 18+
- Fluency in Hmong and English (writing, listening, speaking); Spanish or Portuguese a plus
- Strong interpersonal communication skills
- Self-motivated with the ability to work independently with minimal supervision
- Familiar with Google Suite (Drive, Doc, Sheets) and Video Conferencing (Zoom, Microsoft Teams, Webex)
- Ability to navigate client service software and Microsoft Suite (Word, Outlook, Excel, PowerPoint)
- Reliable transportation
- Integrity and professionalism
- Cultural understanding of Hmong and Hispanic traditions and sensitivity to diverse communities
