Clinical Magnet, a division of ICON Medical Network, is seeking a passionate, relationship-driven Outreach Specialist to support a growing PACE (Program of All-Inclusive Care for the Elderly) program in the Denver, Colorado area.

What You'll Do
You'll build and maintain referral partnerships with healthcare providers, hospitals, rehabilitation facilities, senior living communities, and community organizations while educating prospective participants and referral sources about PACE services. You'll manage your own territory, maintain an active sales pipeline, and help drive enrollment growth throughout the Denver market.
Key Responsibilities
- Develop and grow referral relationships throughout the Denver community
- Execute territory sales and business development strategies
- Conduct presentations and educational meetings with referral partners and prospective participants
- Prospect new referral sources through networking and community outreach
- Manage sales activities and documentation within Salesforce CRM
- Maintain consistent follow-up and relationship management
- Meet or exceed monthly enrollment goals
- Represent the organization at community events and networking opportunities
Qualifications
- Experience in healthcare sales, business development, or community outreach
- Background in home health, hospice, senior care, post-acute care, managed care, or related healthcare environments preferred
- Proven ability to develop referral relationships and grow a sales territory
- Excellent communication and presentation skills
- Experience with Salesforce or other CRM platforms preferred
- Valid driver's license with the ability to travel throughout the Denver area
