Full Job Description

CASE MANAGER

Summary of Job Responsibilities:

- Implements the Options Counseling component of the Aging and Disability Resource Center and administers other programs within the ADRC as needed.

- Serve as Case Manager to the Gibson County Deaconess County clinics to provide education and community resources to clinic patients.

Essential Duties and Responsibilities include the following: (Other duties may be assigned.)

Program Responsibilities:

- Collect and document individual socio-demographic information, initiate appropriate follow-up for the individual, and monitor resolution status of each request.

- Serve as a resource specialist by linking clients to services available through community agencies and organizations.

- Serve as an expert in long-term care services by providing information and direction to individuals and family needing access to services and resources.

- Complete initial assessments for services for AAA funded services (CHOICE, Title IIIB, Title IIIE, and Title IIIC2) in a person-centered framework to stabilize long-term support to individuals and their families in times of immediate need and provide ongoing support.

- Complete assessment of services for individuals who are transitioning from various care settings including but not limited to; ER, hospital, rehab facility, assisted living facility, long-term care facility, and back home.

- Develop person-centered services plans by conducting a face-to face interview to discover strengths, values, and preferences of the individual.

- Assist individual with obtaining Medicaid.

- Complete all required records, case narratives, forms and reports accurately and submit within the specified time frames in the state approved software.

- Assist with phone coverage for all calls concerning the ADRC, including coverage during holidays, lunch, and/or all staff training on an as needed basis.

Community and Public Relations:

- Maintain an effective professional relationship with community resources, providers of service and other staff.

- Facilitate Generations’ involvement in community activities, public meetings or other similar functions and serve as the agency representative at these functions as assigned.

- Facilitate outreach, including presentations and consultations, in the community to enhance case management services.

- Spend time building community resources in the form of volunteer supports by providing presentation to local social service agencies, government agencies, and/or churches as needed or requested by leadership.

Qualification Requirements: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. An individual must adhere to the guidelines of confidentiality in all aspects of job performance from clients, vendors, co-workers and employer. Within six months of initial employment, must complete the Case Management certification through the Division of Aging. Also required to obtain 20 hours Division of Aging approved training a year. May be requested to obtain additional certifications, such as SHIP. The requirements listed below are representative of the knowledge, skill, and/or ability required. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

Education and/or Experience:

A Qualified Intellectual Disabilities Professional (QIDP) who meets the QIDP requirements at 42 CFR 483.430

Possess a degree or licensing as described below:

o An associate’s degree in nursing; or

o An associate’s degree in any field with a minimum of four-year full-time direct service experience with the elderly or disabled (this experience includes assessment, care plan development, and monitoring); or

Bachelor's Degree in any field; or

A license as a Registered Nurse with one year of experience in human services; or

A Master's Degree in any field.

Miscellaneous:

Strong management, communication, planning and organizing skills required.

Ability to use computers.

Must have reliable vehicle and valid driver’s license.

Must have evidence of auto insurance coverage 100,000/300,000 effective March 2016.

Must have a negative TB test or chest x-ray.

Must pass a current limited criminal history check.

Physical Demands:

- Regularly required to talk or hear and taste or smell.

- Required to sit for 1-2 hour period of time.

- Must use hands and/or fingers to feel or handle tools, objects, controls, and reach with hands and arms.

- Must physically be able to meet the demands of home visits.

- Must occasionally lift and/or move up to 40 pounds.
