Case Manager- Mobile Crisis Team
This position is located at our Crisis Center in Dothan, AL
Saturday-Sunday 11pm-7am
$2 Shift Differential
POSITION SUMMARY/FUNCTION
Responsible for providing case management services to seriously mentally ill and/or substance use individuals of the Crisis Stabilization Unit and/or in the community via Mobile Crisis Teams. Position will be responsible for completing and implementing case management plans, working with community partners (law enforcement agencies, etc), and linking individuals with community resources and continuing behavioral health care. Will be responsible for maintaining appropriate records. Travel will be required.
ESSENTIAL DUTIES/RESPONSIBLITIES/FUNCTIONS/TASKS:

Coordinates and provides case management services to individuals within the community via Crisis Mobile Team.

Coordinates admissions, discharges, and follow up services for individuals seen through the Crisis Stabilization Unit, Mobile Crisis Team and/or other programs as assigned.

Will work with the Mobile Crisis Therapist to provide contact to individuals and direction in seeking services or placing them in appropriate services to include temporary observation, extended observation, etc.

Responds to crisis intervention emergencies as needed within the facility and/or in the community minimizing self-harm and/or harm to others.

Will work with law enforcement regarding individuals in the CSU and/or community with goals to minimize potential commitment, hospitalization, while providing care in the least restrictive environment.

Complete case management needs assessments and develop a case management plan;
-
Implement plan by providing case management services. May provide basic living skill, psychoeducation, medication knowledge, etc to those in need.

Advocate individual’s needs with community providers and provide linkage to community services;
-
Monitor progress of individuals and provide follow-up services as needed.

Performs administrative functions in support of case load.
-
Using required case management forms, determines through assessment individual’s assets, deficits and basic needs; reviews needs and individual’s progress on a regular basis.

Works with the Mobile Crisis Therapist to coordinate with community agencies and programs, such as half-way houses, Stat Employment Service, VRS and DHR, the delivery of substance use services, etc.

Will serve as part of the clinical treatment team, participating in case staffing sessions as scheduled.

Completes documentation and services in accordance with all requirements of SpectraCare Clinical Policy, including but not limited to Alabama Administrative Code, CMS, and all other applicable regulating and contractual entities. Completes in a timely manner in the EMR system.

Develops and implements case management service plan for individuals.

Consult with referral source(s);
-
Assist with developing client goals which may include but not limited to personal grooming, skills teaching, medication knowledge, and financial resource assistance, etc.
-
Coordinate service delivery to individuals among appropriate agencies as specified in the service plan.
-
Provide family support and/or basic living skills as needed.
-
Conduct periodic assessment and modification of service plan.
-
Advocate on behalf of individual and target population as a whole regarding social, and/or economic requirements.
-
Conduct home visits and/or other visits within the community.
-
Provide transportation only when it is a necessity. Ensure all community resources such as family, friends, public transportation and special vehicles are utilized to meet the needs of individual. If transporting individuals in personal vehicle, must maintain additional liability insurance as required.

Coordinate care for individuals between agencies and monitor/follow up with the individual and agency regarding this care.

Close cases as appropriate.
-
Meet required productivity level of direct service time.
-
Represents the needs of persons with serious mental illness and co-occurring substance use disorder issues.
-
Establish liaison with other community resources;
-
Identify service gaps;
-
Advocate for the individual;
-
Provide transportation as needed and when appropriate.

Assist with maintaining individual’s records in a professional manner and in accordance with all SpectraCare policies and procedures, Alabama Administrative Code, CMS, and all other applicable regulating and contractual entities.

Ensure privacy and confidentiality of all individual’s records in accordance with HIPAA, 42 CFR and all other applicable state or federal laws.

Ensure all written documentation to include but not limited to SUN-R assessments, individualized service plans, etc is completed accurately and in a timely manner.

Ensure that all records are staffed within standard time frames, with service plan revised as needed and in accordance with regulatory requirements.

Ensure that all progress notes are completed promptly, written in a professional manner and in accordance with policies and procedures.
-
Ensure that goals are appropriate, specific, and the focus of treatment.

Ensure that all recipient information is kept up to date.
-
Ensure recipient release of information forms are signed in a timely manner and maintained within the chart.
-

Serve as a liaison and advocate for SpectraCare individuals with community agencies and the public.

Assist with ensuring the care coordination/case management services remain within budget parameters.

Participates in beginning/end of shift unit reporting.

Fulfill duties of Nurse Delegation Program.

Become fully certified as a Medication Administration Certified (MAC) assistant.
-
Follow all regulations as outlined in the MAC policies including but not limited to the Seven (7) Rights and Responsibilities, training, consultation with RN/LPN, error reporting, etc.
-
Comply with all documentation requirements of the MAC program including the MARS, counting and recording controlled substances, PRN medications, sign out sheets for home visits, and medication errors reporting sheets.

Regular attendance with timely arrival and departure according to scheduled hours.
-
Must be able to work full time with rotating assignments as necessary to include but not limited to overtime hours, nights, and/or weekends as scheduled.

To ensure recipient/employee safety while on duty, must remain awake during scheduled working hours.

Comply with SpectraCare’s attendance policy.

ESSENTIAL QUALIFICATIONS/KNOWLEDGE/SKILLS/ABILITIES:

Knowledge of psychiatric and substance use disorders in the adult population and/or knowledge of addiction as a primary/secondary illness.
-
Knowledge of cultural diversity
-
Knowledge of behavior management, crisis intervention, and interventions common to acute psychotic individuals and/or individuals with substance use issues as well as , to non-violent crisis intervention practice.

Knowledge of available community resources.

Knowledge of educational methods appropriate to adults with severe and persistent psychiatric disorders

Ability to design, implement/administer individualized behavioral services in regard to case management services.

Ability to provide services in all facilities, in the community and other locations as deemed necessary.

ESSENTIAL TRAINING REQUIREMENTS:
-
Complete initial new hire training.
-
Complete required on the job training (OJT) training by the department.
-
Obtain proper certifications needed to perform job duties.
-
Complete annual training as required by the Alabama Administrative Code, CMS, and all other applicable regulating and contractual entities.
-
Complete Care Coordination / Case Management Training as required by the Alabama Department of Mental Health
-
Complete First Aid/CPR upon hire and prior to working alone with clients; and continuously thereafter maintaining current certification.
WORK ENVIRONNMENT / PHYSICAL REQUIREMENTS:
-
Work location is primarily in a temperature-controlled office with extended periods of computer use at a desk with associated repetitive motions. Must understand and practice good ergonomic movements to avoid injuries.
-
In being placed on Mobile Crisis Team, will travel within the community to provide services to individuals in need and may be exposed to various weather conditions.
-
Must be able to travel to system facilities with exposure to changing weather conditions.
-
There is an occasional need to maintain back extension while being on stomach with one’s own body weight on hips and legs. This position is maintained for several minutes.
-
Occasionally, be able to put one’s own body weight on one knee and one elbow on the same side of one’s body.
-
Occasionally, be able to kneel and reach to same side toward the floor.
-
Strong grasp is needed to open doors throughout the facility, and to maintain constant contact with an individual during a restrain procedure or with other hold techniques.
-
There are times that require the employee to be able to ambulate as quickly as possible to assist coworkers with an individual in a restraint procedure.
-
Employee may be exposed to one or more of the following: disagreeable odors, chemicals such as disinfectants, cleansers, soaps, etc; latex, plastic and other materials used for personal protective equipment; noise and distractions; unpleasant sights; unpredictable behaviors; body fluids; contagious diseases; and/or wet floors.

Job requires occasional use of office equipment and ability to correctly lift up to fifty (50) pounds on an infrequent basis.

Job requires occasional and limited bending, twisting, lunging, squatting, and reaching.

Required to talk to or hear staff, clients, and/or others.

Physical ability to perform CPR.

Required to train professional staff through verbal and visual presentations.

REQUIRED QUALIFICATIONS/EDUCATION/EXPERIENCE:

Bachelor’s degree in psychology, social work, human services and/or related field.

Must have a valid driver’s license.

Good MVR acceptable to automobile insurance carrier for inclusion on agency policy.

Must maintain automobile liability insurance as required.

Willing to complete Case Management training upon hire.
PREFERRED QUALIFICATIONS/EDUCATION/EXPERIENCE:
