CASE MANAGER - RESIDENTIAL
JOB SUMMARY:
The Case Manager maintains all regulatory and contract issues by the individual served by The Arc SWGA. Identifies report and corrects any environmental, documentation, medical/safety issues for individuals served by The Arc SWGA Residential services. Maintains a checklist/tracking system for any issues identified for up to 25 individuals residing in the assigned area.

DUTIES AND RESPONSIBILITIES
Conducts at least one (1) face-to-face home inspection for individuals receiving residential services per month (i.e., when the individuals are at the home and could be nights and weekends). Checklists and all approved forms must be completed and signed by the individual and contractor; all findings must be discussed. All forms must be completed in Therap by the 5th of the month for the previous month.
Identifies issues and completes request forms as needed and completes follow up within two weeks.
Maintains organized record keeping system for all reports and supporting documentation.
Completes in-home file audit one time per month.
Conducts audits on petty cash logs and reports any issues immediately.
Maintains responsibility of food stamp card and turn in receipts monthly.
Ensures fire/safety drills are conducted and forms completed within regulatory time limits. Ensures these records are maintained in an organized file at each residence.
Maintains communication with assigned nursing staff in reference to any and all medically related information for the individuals on assigned case load (med changes, physician appointments, dental appointments, change in health status, any behavioral and/or medical changes).
Schedules and facilitates all ISPs ensuring that all involved professionals are invited.
Attends all SIS, ISP meetings and addendum's for individuals on assigned case load. Ensures that all pertinent information is presented and included on the ISP.
Collects, reviews and scans in all medical and dental forms, labels appropriately and forwards to Residential Director, LPN/HRST and Nursing Department.

Tracks ISPs and upon approval ensures that all documentation and tracking logs are placed in the files of the individuals. Ensures HRST thirty-day training is completed in the allotted time. Documentation to be maintained in the home and main file.
Maintains contact/communication with support coordinator and family members, ensuring all are aware of any issues that arise with the individual.
Ensures Direct Support Professional working with the individuals maintain current training and certifications as required per DBHDD/HFR/AARC and Medicaid rules, regulations, and policies. (Direct Support Professional may not have contact with the individuals unless 100% certifications and training requirements are current.)
Documents any issues with the Direct Support Professional and makes recommendations to Management Team about possible new placements, reassignments, or discharges.
Demonstrates working knowledge regarding the sections of community service standards for all providers referencing individuals receiving waivers or state-funded services. Ensures working knowledge of PHC/HFR rules and regulations. Demonstrates expertise in critical incident reporting policy and guidelines. Ensures all license and required documentations and certifications are present and/or posted. (Evacuation plan, waiver standard, FLA poster, etc.)
Develops a tentative weekly schedule to meet all deadlines. Schedules must be approved by Residential Director. Email or call Residential department by 8:00 AM each working day to report absence if not in the office.
Reviews support coordination ratings every Friday morning prior to Residential meeting. Develops tentative action plan to be presented at the weekly departmental meetings. Ensures completion CAP. Documents CAP actions and forwards to QI.
Conducts unannounced visits at random times to ensure best possible health and safety of the individuals served.
Maintains human and civil rights of all individuals served by The Arc of SWGA. Reports any suspicious activities or concerns immediately.
Participates in one The Arc of SWGA committee.
Provides a detailed report, verbal or written, in reference to the individuals on assigned case load to be presented at the weekly departmental meetings.
Completes all staff training as well as other identified training.
The following training courses are required by The Arc of SWGA and must be completed annually to maintain compliance:
Carries and answers cell phone calls promptly and returns call promptly according to urgency of call. Listens to voice mails ongoing.
Reads and responds to emails prior to completion of work day.
Takes calls as assigned; may need to stay in home if no staff is located.
promotes and protects individuals’ rights, especially the right to freely make and express preferences in choices regarding their daily living and care, as long as such rights/choices are not detrimental to individuals’ well-being.
Other duties as assigned.

QUALIFICATIONS:
Required: a four (4) year degree from an accredited institution in Social Work, Education, Nursing and/or other related field plus 1 year experience in the disability field. All skills and experience will be considered. Must have knowledge of calculators, personal computers, electronic financial data systems, MS Office Excel, fax machines, multiple line telephone systems and all other business related machines. Must have experience in clerical/office administrative duties. Must be extremely organized, be an effective communicator (both verbal and written), be a team player, be able to coach, train, and hold staff accountable and produce reports in a timely manner. Must be a self-starter, be able to solve problems and provide creative suggestions and solutions. Must have excellent customer service skills.
