Job Summary
We are seeking a dedicated and compassionate Hospice Resource Registered Nurse (RN) to join our dynamic healthcare team offering comprehensive care in homes or facilities. This is a hybrid position with responsibilities ranging from conducting admissions, managing patient caseloads, covering on-call schedules or any combination therein. In this vital role, you will serve as a clinical resource, providing expert guidance and support to hospice patients, their families, and interdisciplinary team members.

Key Job Description Responsibilities

Admissions & Assessments: Perform initial home/facility assessments, determine hospice eligibility, obtain informed consent, educate patients/families on hospice services and hospice admissions.
Case Management: Develop and update individualized plans of care, manage medication, coordinate with the interdisciplinary team (IDT), and visit patients to ensure comfort and pain control.
On-Call & Resource Support: Trained to cover on-call shifts. Participate in the resource RN on-call rotation for emergency visits and phone triage, covering weekends or evenings to provide 24/7 care support.
Documentation: Maintain accurate, timely electronic medical records in compliance with state/federal regulations.

Duties

Key duties include hospice patient care with compliant documentation, comfort and symptom management, assessing eligibility, providing education on end-of-life care, providing emotional support to families and on-call triage and patient visits.

Act as a clinical resource for hospice staff by providing education, guidance, and support on complex patient cases and clinical protocols
Collaborate with interdisciplinary teams to develop and implement individualized care plans that meet each patient’s unique needs
Conduct comprehensive assessments of patients’ physical, emotional, and physiological status to inform appropriate interventions
Provide expert consultation on medical documentation, medication management, and symptom control strategies
Educate patients, families, and staff on topics such as physiology, anatomy, medical terminology, HIPAA compliance, and end-of-life care practices
Assist with training and development initiatives to enhance staff competencies in hospice care delivery
Ensure all documentation complies with regulatory standards and maintains accuracy in medical records

Required Qualifications & Skills

Licensure: Active, unencumbered RN license.
Experience: Minimum 1-2 years experience as a nurse, with hospice or home health experience highly preferred.
Skills: Expertise in symptom management, excellent communication skills for family education, and ability to work independently, often for companies like UnitedHealth Group or local providers.
