Full Job Description

POSITION TITLE : Case Manager – Justice Programs

LOCATION : District Attorney's Office

HIRING RANGE : $61,315 - $68,128/Yr.

STATUS : Full Time

HOURS PER WEEK : 40

WORK HOURS : 7:30am-4:30pm

DEPARTMENT: District Attorney’s Office

PAY BASIS: Salaried

PURPOSE AND SUMMARY :

Provides professional case management, assessment, and service coordination for individuals involved in justice-system programming.

ESSENTIAL FUNCTIONS:

Interviews and assesses program participants using various evidence-based risk and needs assessment tools and reports assessment results and findings to District Attorney’s Office attorneys.

Performs case management duties, including monitoring participant progress toward completion of court-ordered conditions, meeting with participants on a regular basis, referring participants back to Court for revocation as appropriate, and assisting participants with the program discharge process.

Assists participants with addressing needs by referring to appropriate resources, tracking service connections, and monitoring follow-through with appointments and services.

ADDITIONAL ESSENTIAL DUTIES:
Performs case management responsibilities in a timely and efficient manner as evidenced by complete and up-to-date records on individuals for whom the case manager is assigned.
Coordinates interagency care and communicates with treatment teams, healthcare providers, courts, law enforcement, and socioeconomic support providers.
Compiles service plans for assigned participants based on risk and needs assessment tools.
Ensures that participants are treated courteously and are provided or referred for appropriate services.
Updates the color hotline for urine analysis (UA) conditions daily and the monthly calendar in advance to ensure participants know when to submit UAs and that the color hotline process runs smoothly.
Prepares confidential correspondence and maintains confidentiality when required.
Drafts, files, and distributes documents with courts and other entities as required or directed.
Works with Victim Witness Unit to ensure compliance with victims’ rights.
May be assigned to Drug Court, bond monitoring programs, diversion programs, or other similar initiatives per the needs of the Connect Program.
Testifies in court as required.
Assists the District Attorney, Deputy District Attorney, and Special Programs Supervisor with development of new programs, including new conditions and participant resources, and the updating of current diversion contracts.
Plans and organizes own work to achieve program objectives.
Maintains skill and competence levels by attending appropriate meetings and continuing education trainings as policies and regulations change.
Performs other related duties as may be assigned.

WORK RELATIONSHIPS:

Reports to Special Programs Supervisor.
Works regularly with District Attorney’s Office employees and community partners.

EDUCATION, EXPERIENCE, TRAINING, AND CERTIFICATION:

Bachelor's degree is required, preferred in human services or criminal justice.
Two or more years experience in a social service or criminal justice field preferred.

KNOWLEDGE, SKILLS, AND ABILITIES:

Knowledge of resources and their availability as well as their application to participants and families.
Considerable ability to gather information, organize facts and clearly present them, verbally and in writing, to attorneys, judges, and participants.
Knowledge of goal-oriented counseling and motivational interviewing techniques preferred.
Possess good organizational skills and considerable ability to multitask.
Ability to maintain strict discretion and confidentiality of information as well as the identities of certain persons, roles, and operational matters.
Ability to effectively address and resolve conflict and to manage stress in a professional manner.
Ability to use and learn various computer software applications.
Ability to file information properly and consistently.
Ability to plan, organize, and perform duties, including case management, with limited supervision.
Ability to work a flexible schedule as workload dictates.
Ability to make decisions in accordance with rules of ethics, laws, and court procedures.
Ability to deal effectively and cordially with the public.

PHYSICAL REQUIREMENTS:
Ability to perform most work from a sedentary position, sit for extended periods of time, frequently move throughout the office.
Ability to function in situations encountered in a normal office setting.
Ability to use standard office equipment including telephone, computer, printer, photocopier, and scanner.
Ability to travel to other County departments and locations.
