General Statement of Duties:
-
To advance the Mission Health System Patient Safety Program through the promotion of a culture of safety and implementation of strategies for the elimination of avoidable patient harm. These efforts include the standardization of demonstrated safety practices and the sharing of patient harm events for transparency and proactive risk assessments across the enterprise.
Minimum Education Required:
-
Bachelor’s degree in healthcare related field required.
-
Master’s degree preferred.

Clinical background preferred.
Experience Required:
-
Five to seven years healthcare experience in patient safety, risk, and/or quality required.
-
Healthcare experience should be recent and within a clinical setting such as hospital, ambulatory surgery center, etc.
-
Program oversite at a system level for multiple facilities is a plus.
Minimum License, Certificates, or Registrations Required:
-
Certified Professional in Patient Safety (CPPS), required. Maintain CPPS while in role.
-
Certification in Healthcare Risk Management (CPHRM) or Certified Professional in Healthcare Quality (CPHQ) is preferred.
Essential Job Functions:
Systems Thinking and Reliable Design Expectations:
-
Support prevention of future harm by initiating and overseeing proactive evaluation and redesigning organizational systems to improve care processes (e.g. force functions, checklists, error causation thinking, human factors, applied informatics, Just Culture and safe tables).

Support improved outcomes by emphasizing both appropriate behaviors and robust systems that include concise accountability measures and follow-up.
-
Promote consistent delivery of evidence-based care and reduction in preventable harm by focusing on applying the principles of reliable design.
-
Collaboration with system leaders to include nursing, medical staff, information technology, pharmacy, graduate medical education, process improvement, service lines, the HCA Patient Safety Organization (PSO) and others as appropriate to explore identified variations, complex systems and improvement strategies.
-
Utilize PSO alerts and best practices (e.g. The Joint Commission Sentinel Event Alerts) to perform gap assessments and implement strong actions that will alleviate identified gaps within system facilities.
Identification and Mitigation of Patient Safety Risk Expectations:
-
Oversee and consult with facility PSDs to ensure effective reporting, investigation, and analysis of patient safety events/errors and potential risks in the facility.
-
Review serious event analyses for thoroughness and credibility and strong sustainable improvement strategies and provide feedback and education to team members.
-
Educate on thorough and credible failure mode effect analysis (FMEA) to identify and mitigate unintended adverse patient outcomes and evaluate effectiveness of process changes as a result of facility or system risk assessments.

Partner with facility PSD to perform Patient Safety Rounds that identify patient safety risks and empower staff to identify and participate in the resolution of patient safety concerns.
-
Educate PSD on disclosure of serious events to patients and/or families in accordance with organizational policy and regulations.
-
Assure timely reporting of Patient Safety Work Product (PSWP) to the PSO.
-
Actively lead system wide PSO safe tables/learning collaboratives and ensure implementation of best practices, alerts, and updates to drive patient safety improvement.
-
Ensure implementation and/or updates of patient safety driven policies from the HCA enterprise.
-
Ensures facility-based responses, included documented analysis and actions taken on all PSO Alerts (audits, direct observation, site visits, data review).
-
Occasionally, attends hospital level Serious Event Analysis (SEA) to ensure proper execution of the SEA as well as identification of potential and actual root cause, identification of immediate mitigating actions.
Safety Culture Advancement Expectations:
-
Ensure analysis of the culture of safety (AHRQ, Joint Commission and Leapfrog) results in data-driven action plans that lead to targeted improvement and outcomes.
-
Support and encourage patient harm reporting throughout the organization through a nonpunitive relationships between leadership and staff.
-
Provide an avenue for system transparency and sharing of patient events to encourage facility awareness of risks.
-
Facilitate thorough and credible review of events that address both system and individual accountability.
Measurable Reduction in Avoidable Harm Expectations:
-
Oversee the management and use of event information to benchmark and track progress to zero avoidable harm across Mission Health.
-
Provide high-level analysis and identify trends from reports (e.g. event reports, mortality reviews, HACs, Service Line Dashboards) to track progress of improvement strategies, promote positive spread and sustainment in improvements.
-
Provide Mission Health level coordination and oversite of Mission Health wide initiatives to reduce the overall rate of harm.
-
Primary oversight of Clinical Safety Improvement Program (CSIP) and all deliverables; partners with clinical, nursing or ancillary leaders on CSIP.
Risk Management/Claims Activities:
-
Work proactively with HCI Centralized Claims to identify trends and opportunities for Mission Health facilities.
-
Oversees document requests for preparation of HCI legal cases in coordination with the facility PSD.
-
Collaborate with facility PSD on non-HCI cases as needed.
-
Collaborate with HCA Operations Counsel and PSO on potential harm issues and for guidance to support Mission Health patient safety and response.
-
Coordinates contract reviews with appropriate contract owners for appropriate patient safety metrics.
Partnership with Executive and Clinical Leaders Expectations:
-
Collaboration with Mission Health system clinical education team to provide patient safety/risk management content in new hire orientation and/or curriculum presentations.
-
Provide ongoing education to Mission Health system and facility leadership on the science of safety (high reliability, effective communication, sustaining awareness/alertness) and patient safety initiatives/adverse event trends.
-
Collaborate to provide patient safety content to System wide development programs, e.g. Team STEPPS.
-
Collaborate with System General Medical Education (GME) leadership on patient safety training for all residents.
-
Collaborate with facility leaders and managers to ensure thorough, credible and timely event management.
-
Partner with facility leaders to identify and hardwire behavioral norms that promote a culture of safety.
-
Partner with facility leaders to ensure understanding of and compliance with the National Patient Safety Goals.
-
Partner with facility leadership to establish activities that enable and sustain an open and fair environment promoting learning, safe systems, and appropriately managing behavioral choices related to patient safety (e.g. Just Culture algorithm, Patient Safety Rounds, Event Response, Disclosure).

Partner with facility VPQs to complete the NQF Safe Practices section of the Leapfrog Hospital Survey as applicable.
Patient and Family Engagement Expectations:

Encourage facilities to engage patients/families when appropriate in the patient safety program and seek input from patients/families involved in harm and close call events as appropriate.
Leadership:

Serve as a mentor and subject matter expert to PSDs within Mission Health and provide patient safety onboarding and guidance.

Demonstrate knowledge of the occurrence reporting system and serve as an expert to others.

Ensure the coordination of communication to and from the System Quality Team regarding patient safety projects/activities throughout Mission Health.

Provide relevant and timely feedback to PSDs and Market VPQ to improve processes.

Serve as Mission Health liaison with the Corporate Patient Safety Department, legal counsel and HCI.
-
Contribute to the attainment of business goals through positive impact on continuous quality and performance improvement outcomes of publicly reported patient safety data.

Demonstrate personal responsibility for professional development and actively promotes development of self and others through participation in professional organizations, educational opportunities, etc.
Knowledge/Skill/Abilities:
