Graduate of an accredited school of professional nursing, BSN Preferred
Experience / Qualifications

RN- Current RN licensure in the state of Florida

American Heart Associations BLS Certification for Healthcare Providers

Five (5) years of nursing experience with three (3) in Oncology related field preferred

Must have knowledge of physician office processes.

Must have excellent verbal and written skills.

Must be proficient in building relationships and in coordination of all aspects of care.

Knowledgeable of oncology disease process and symptom management.

Ability to foster and maintain effective working relationships and communication levels with patients, employees, physicians, administration and the general public.

Position Summary
The Nurse Navigator will serve as a single point of contact for referring physicians, patients, and caregivers to provide resources and assistance to patients. They will assist in overcoming treatment barriers while expediting access to clinical and supportive care services offered within the Jupiter Medical Center’s Program, and the community.

The Nurse Navigator will prepare and maintain an updated treatment plan for each patient to synchronize care management between the medical team and patient.

Assistance with patient appointments, including those made with labs, diagnostic facilities, and specialty physicians will be provided.

Community outreach including possible public speaking and promotion of the program and Jupiter Medical Center system is an essential part of this position.

Physical Requirements
This job can be physically demanding at times requiring stooping, sitting, standing, and walking for long periods of time. The team member will need to frequently assist lifting patients and heavy equipment. Must be able to work in a stressful environment, work independently, making sound decisions, detail oriented, alert, and self-motivated. Ability to push/pull up to 40lbs, carry and lift up to 50lbs.
