Full Job Description

POSITION SUMMARY:

This full-time position is directly involved in a collaborative process planning, facilitation, and advocacy for options and services to meet a patient’s health needs across settings that span the continuum of care (i.e. post hospitalization, skilled nursing, acute rehab and home). Palliative Care needs are identified through assessment, risk of readmission, data reporting and collaboration with hospital discharge planners, attending and palliative care physicians and advanced practice providers (APPs). The Palliative Care Home Care RN supports the team in achieving clinical standards across the continuum of hospital, clinic and community settings, coordinates resources, and provides acute and palliative nursing care under the direct supervision of the Palliative Care Medical Director.

ESSENTIAL JOB FUNCTIONS:

Responsibilities include but are not limited to:

Provide in-home assessments, re-assessments and support to Palliative Care Program patients (lab draws, wound care, minor procedures, immunizations, etc.…) under the direct supervision of the Palliative Care Medical Director.
Provide accurate and complete medical records documentation within 48 hours of a home care visit.
Engage community resources at the direction of the Primary Care Provider (MD), Advanced Practice Practitioners (APP), and Licensed Masters Social Worker(s) (LMSWs), that are timely and cost-effective to obtain optimum value for the patient in support of medical and social determinates of health.
Triaging the Primary Care Provider (MD), Palliative Care Provider (MD) and advanced practice practitioners (APPs) patient phone calls and relays information to the MD, APP, or office nurses.
Uploading MOLST forms, health care proxies, and medical images to Medent, as necessary.
Confirm Palliative Care appointments at least three days prior to a visit.
Ensure that all clinical documentation is complete daily.

REQUIRED QUALIFICATIONS:

Registered Nurse (RN) with active New York State License
5 years minimum in nursing with 3 years case management, home health care and palliative care preferred
Experience with geriatric population preferred
Must demonstrate strong verbal and written communication skills
Must demonstrate ability to work as part of the team

SKILLS AND ABILITIES:

1. Ability to be a team player and exercise initiative in responding to provider requests and concerns in a

helpful and courteous manner.

2. Provide customer service techniques.

3. General understanding of the health care industry, ideally the provider relations field-desired

4. Excellent interpersonal and telephone communication skills are required.

5. Exercise attention to detail; ability to provide accurate data entry.

6. Ability to work under pressure with minimal supervision, multi-task, complete projects in a timely

fashion and meeting deadlines a must.

7. Excellent analytical, problem solving and organizational skills a must.

8. Language Skills: Strong communication skills both written and verbal to work with multiple internal and external clients in a fast-paced environment.

9. Reasoning Ability: Ability to apply principles of logical or scientific thinking to a wide range of

intellectual and practical problems.

10. Computer Skills: Ability to create and maintain documents using Microsoft Office (Word, Excel,

Outlook, PowerPoint)
