Full Job Description

Join our compassionate and resident-focused team as a Wellness Nurse, where your clinical expertise and leadership will play an essential role in supporting the health, safety, dignity, and overall well-being of residents in Assisted Living and Memory Care.

The Wellness Nurse provides hands-on nursing support, assists with resident assessments and care planning, oversees medication management and clinical documentation, and collaborates closely with residents, families, physicians, pharmacy partners, caregivers, medication technicians, and other members of the interdisciplinary team.

This role is ideal for a nurse who is passionate about caring for older adults, including individuals living with Alzheimer's disease and other forms of dementia, and who thrives in a collaborative environment focused on promoting independence, quality of life, and person-centered care.

Responsibilities

Conduct comprehensive resident assessments, including vital signs, health history reviews, functional observations, cognitive status, changes in condition, and other clinical evaluations within the nurse's scope of practice.
Assist with the development, implementation, and ongoing review of individualized service plans and care plans based on each resident's needs, preferences, abilities, and level of care.
Monitor residents for changes in physical, cognitive, emotional, and behavioral condition and ensure appropriate assessment, documentation, intervention, physician notification, and family communication.
Administer medications and treatments as permitted by applicable state regulations, the Nurse Practice Act, physician orders, and community policies and procedures.
Support medication management processes, including medication reconciliation, review of physician orders, pharmacy coordination, medication cart audits, and oversight of medication administration records.
Provide clinical guidance and support to caregivers and medication technicians to help ensure resident care is delivered safely, consistently, and in accordance with individualized service plans.
Collaborate with physicians, nurse practitioners, pharmacies, home health providers, hospice agencies, rehabilitation professionals, and other healthcare partners to coordinate resident care and promote positive outcomes.
Communicate proactively and compassionately with residents and families regarding health status, changes in condition, care needs, and recommended interventions.
Participate in resident care conferences and interdisciplinary team meetings to promote continuity of care and ensure resident needs and preferences are appropriately addressed.
Support residents living with Alzheimer's disease and other forms of dementia through person-centered approaches that promote dignity, independence, choice, comfort, and meaningful engagement.
Recognize and respond appropriately to changes in condition, falls, injuries, behavioral changes, medication concerns, and other clinical events, following established notification and documentation requirements.
Provide first aid, CPR, and emergency assistance as appropriate, including the use of an AED when trained and required.
Maintain timely, accurate, and complete clinical documentation within the Electronic Health Record (EHR) and other required systems.
Assist with infection prevention and control practices, including monitoring for signs and symptoms of communicable illness and reinforcing appropriate precautions.
Maintain resident confidentiality and comply with HIPAA, applicable assisted living regulations, the State Nurse Practice Act, and organizational policies and procedures.
Participate in quality assurance, clinical audits, regulatory compliance activities, incident investigations, and performance improvement initiatives as assigned.
Promote a culture of resident dignity, safety, accountability, teamwork, and compassionate service throughout the community.

Requirements

Current and unrestricted nursing license in good standing with the applicable State Board of Nursing. LPN/LVN or RN licensure may be accepted based on state regulations and community requirements.
Previous nursing experience in assisted living, memory care, long-term care, skilled nursing, geriatrics, home health, or a related healthcare setting preferred.
Experience caring for older adults and individuals living with Alzheimer's disease or other forms of dementia strongly preferred.
Knowledge of medication administration, resident assessment, care planning, infection prevention, changes in condition, and clinical documentation.
Familiarity with applicable assisted living regulations, HIPAA requirements, State Nurse Practice Acts, and nursing standards of practice.
Proficiency with Electronic Medical Record (EMR) or Electronic Health Record (EHR) systems and the ability to maintain accurate, timely clinical documentation.
Current CPR and Basic Life Support certification as required by state regulation and organizational policy.
Strong clinical judgment and the ability to identify changes in resident condition and respond appropriately and promptly.
Excellent communication and interpersonal skills, with the ability to build positive relationships with residents, families, team members, healthcare providers, and community partners.
Ability to provide clinical guidance and education to caregivers and medication technicians while promoting teamwork and accountability.
Demonstrated compassion, patience, and understanding when supporting residents with cognitive impairment, behavioral expressions, and complex care needs.
Ability to maintain confidentiality, exercise sound professional judgment, and remain calm and effective in urgent situations.
Physical ability to perform the essential functions of the position, including standing, walking, bending, reaching, and assisting with resident care as needed, with or without reasonable accommodation.
