ABOUT US: Health Imperatives (HI) is a multi-service organization dedicated to reducing health and wealth disparities while advancing meaningful opportunities and economic security for people south of Boston. Our mission is to improve the health and well-being of low-income or vulnerable families and individuals in Southeastern Massachusetts. The communities we serve, including Brockton, New Bedford, Plymouth, the South Shore, South Coast, Cape Cod, Martha’s Vineyard, and Nantucket, have some of the highest poverty rates and largest wealth gaps in Massachusetts. For nearly 50 years, we have addressed the complex needs of people who are disadvantaged by poverty and discrimination and have fallen through the cracks of mainstream systems of care. TITLE: WIC Community Coordinator

HOURS: Part-time (20 hours/week); flexible schedule with occasional evenings and Weekends

SALARY: $28.10 per hour

PROGRAM: Cape Cod WIC

BENEFITS: Medical, Dental, Vision, 403 Retirement Plan, Employee Assistance Program, etc.

DUTIES: Under the direct supervision of the Program Director, the Community Coordinator’s responsibilities will include the following:

Develops comprehensive listing, initiate contact and maintains working relationships with health and human service providers, advocacy organizations and other groups who use or might use WIC information in order to facilitate and carry out referrals between eligible populations and WIC.
Develops and maintains the Massachusetts WIC Community Coordinator Database (CCDB) with community contacts made and 60 outreach activities conducted quarterly within the service area.
Facilitates or conducts periodic in-services for staff and/or clients at local health and human service providers and other community agencies that work with the WIC population, i.e., Head Start, nursery or day care, grade schools, churches, EFNEP, neighborhood associations, Housing Authority, etc., as well as local pediatricians, obstetricians and other health care providers.
In coordination with management staff, utilizes the performance management system Plan, Do, Study, Act (PDSA) model for improving outreach and/or retention activities. Submits progress reports, as requested.
Attends monthly staff meetings, Community Coordinators’ Quarterly Meetings, new Community Coordinator Training and In- services, and participates in regional Community Coordinator meetings.
Provides WIC program, education and outreach materials to local agencies and advocacy groups.
In coordination with the program director and state WIC staff, represents WIC on committees or state public health initiatives, as well as other community activities.
Represents WIC at community health fairs, workshops, and other public forums.
Assists with retention activities with particular focus on existing or terminated participants who have already accessed WIC services to continue to participate in the WIC program.
Creates updates and/or manages a local program’s social media platform.

QUALIFICATIONS:

Minimum High School or Associate’s Degree.
Must demonstrate knowledge of community and health and human services in Cape Cod and the Islands region;
must be outgoing, compassionate, and understanding to the needs of participant population;
Good communication skills in oral and written English.
Bilingual Portuguese helpful.
