Position Summary:
As a Certified Nursing Assistant (CNA) at GreenFields, you’ll play an important role in helping our residents feel safe, comfortable, respected, and cared for each day. You’ll provide compassionate hands-on care, assist with daily activities such as bathing, grooming, toileting, meals, mobility, and positioning, and support residents in maintaining as much independence as possible. You’ll also build meaningful relationships with residents and their families while working closely with nurses and the care team to provide high-quality, person-centered care.

Key Responsibilities:
-
Provide compassionate, hands-on care to residents according to their individualized care plans and nursing assignments.
-
Assist residents with activities of daily living, including bathing, grooming, oral hygiene, toileting, dressing, and bedtime care.
-
Safely transfer, ambulate, reposition, and assist residents with mobility while promoting comfort, independence, and safety.
-
Assist residents with meals, feeding, hydration, and nourishment as needed.
-
Provide restorative and rehabilitative care and encourage residents to participate in appropriate self-help activities.
-
Assist residents with appointments, therapy, recreational activities, religious services, and other scheduled events.
-
Respond promptly to call bells and resident requests, providing assistance and communicating needs to the appropriate nurse.
-
Observe residents for changes in condition, behavior, appetite, or other concerns and promptly report changes to the nurse.
-
Assist nurses with treatments, assessments, admissions, transfers, discharges, specimen collection, and other delegated care within the CNA’s scope of practice.

Provide catheter care and other resident care procedures according to facility policies and nursing direction.

Maintain accurate documentation and records related to resident care.
-
Maintain appropriate infection prevention and control practices and follow isolation and safety procedures.

Maintain clean, safe, and orderly resident rooms and work areas.
-
Participate in resident care conferences and communicate observations that support individualized care.
-
Assist residents, families, and visitors in a courteous and professional manner while protecting resident privacy and confidentiality.

Participate in required in-service education and ongoing training.
-
Follow facility policies, procedures, emergency preparedness plans, and safety standards.
-
Promote resident dignity, comfort, independence, security, and overall well-being.
EXPERIENCE: Previous experience in a nursing home or hospital setting is desirable. Must possess a Certified Nursing Assistant Certificate.
EDUCATION REQUIREMENTS: High school diploma or equivalent preferred, but not required.

PHYSICAL AND SENSORY REQUIREMENTS:

Must submit to and pass a yearly examination, PPD skin test, and attend all mandatory in-services.

Must have the physical ability to lift a minimum of 50 pounds.
WORKING CONDITIONS:

ASSIGNMENT FLEXIBILITY AND STAFFING REQUIREMENTS:
-
To best support resident care and team needs, CNAs assigned as Shower Aides may be reassigned to resident care duties before 3:00 p.m., based on staffing needs and at the discretion of the Supervisor or Unit Manager.

Beginning at 3:00 p.m., CNAs serving as Shower Aides will receive a resident care assignment.

Employees will not be expected to perform Shower Aide responsibilities and resident care duties simultaneously. When assigned to resident care duties, expectations will be limited to the responsibilities associated with the assigned role during that period.

Assignment changes may occur during a shift to maintain appropriate staffing levels and meet resident care needs.
