The Mission of the Ripley County Health Department is to promote individual and environmental health, prevent injury and disease in the community, assure emergency preparedness, and preserve vital records. The Purpose of this position is to implement, coordinate, and evaluate programs that promote chronic disease prevention and support the health, wellness, independence, and quality of life of older adults. The position provides health education, coordinates wellness activities, directs the use of community resources, develops partnerships with healthcare and community organizations, and supports initiatives designed to improve health outcomes and increase access to preventive and supportive services. The Coordinator works closely with community members, healthcare providers, senior-serving organizations, other public health agencies and community partners to identify needs, deliver effective health and wellness programs, and connect individuals with appropriate healthcare, social, and community resources.

Chronic Disease Program Duties:
· Develop and promote internal wellness programs focused on chronic conditions, including COPD, diabetes, heart disease, hypertension, obesity, and related health concerns.
· Provide program participants with health education, resources, and practical strategies that promote healthy lifestyle behaviors, improved nutrition, and increased physical activity.
· Help participants identify appropriate community and healthcare resources.
· Coordinate referrals to healthcare, social services, nutrition, physical activity, transportation, and other supportive services as appropriate.
· Promote preventive health practices, including screenings, immunizations, medication awareness, and regular healthcare visits.
Senior Wellness Duties:
· Develop and promote internal programs that promote healthy aging, independence, mobility, and social connection.
· Coordinate wellness activities addressing physical activity, nutrition, fall prevention, and mental and social well-being.
· Work with senior centers, senior housing, assisted living facilities, and other senior-serving agencies to conduct needs assessments and coordinate program implementation.
· Identify barriers affecting older adults' participation in wellness and preventive health programs and develop strategies to improve access.
General Duties:
· Maintain knowledge of and implement evidence-based practices
· Coordinate classes, workshops, support groups, screenings, fitness activities, educational sessions, and community wellness events.
· Develop program schedules, educational materials, participant communication methods, and outreach activities.
· Collect, analyze, and report program participation, satisfaction, and outcome data.
· Assist with grant reporting requirements, developing program budgets, purchasing, and other administrative requirements.
· Ensure programs are delivered equitably and consistently
· Establish and maintain collaborative relationships with healthcare providers, local agencies, nonprofit organizations, and organizations that serve seniors.
· Represent the organization at health fairs, community meetings, senior events, and other outreach activities.
· Promote available programs through community outreach, social media, newsletters, flyers, and other communication channels.
· Identify emerging community health needs and recommend new or enhanced programming.
· Cross train and assist occasionally with additional duties as assigned.
Environmental Conditions:
· Work effectively in an office environment as part of a collaborative team.
· Maintain professionalism when dealing with difficult customers.
· Meet physical requirements including lifting, carrying, and transporting up to 20 pounds and standing for extended periods.
· Travel by personally owned vehicle within Ripley County and other counties in southeast Indiana.
· Able to work in senior centers, senior housing facilities, healthcare centers, and other program settings
· Work occasional evenings or weekends to accommodate community programs and events, flexing weekly hours to compensate.
Skills and Abilities:
· Proficiency in operating standard office equipment, including computers, printers, telephones, copiers, scanners, laminators, and other equipment as required.
· Ability to answer, screen, and transfer telephone calls and respond to emails promptly, professionally, and courteously.
· Ability to read, interpret, and comprehend training materials, policies, procedures, statutes, regulations, and program guidelines.
· Proficiency in creating, editing, organizing, and maintaining documents, spreadsheets, presentations, and databases using Microsoft Office, Google Workspace, Canva, and other applicable software.
· Strong customer service skills, with the ability to respond to inquiries, provide accurate information, assist participants with program-related questions, and make appropriate referrals to outside agencies or community resources.
· Strong initiative and self-direction, with the ability to manage time effectively, prioritize responsibilities, meet deadlines, and proactively engage clients, community members, and community partners.
· Excellent organizational skills, with the ability to maintain, file, retrieve, and manage records and program documentation efficiently and accurately.
· Ability to collect, maintain, analyze, and report program data and statistical information accurately and in a timely manner.
· Ability to identify and monitor statistical trends and use information to support program planning, evaluation, and improvement.

Required Qualifications:
· Valid Indiana Driver’s License
· Vehicle with large cargo space or capable of hauling a small trailer for transporting recreation or exercise equipment
· High School Diploma or equivalent
· Certification or special training in one of the following fields: public health, health education, social work, nursing, gerontology, nutrition, kinesiology, or a related field.
· Experience coordinating or administering programs in public health, community health, health education, senior services, wellness programming, or a related field
· Experience working with individuals with chronic health conditions, older adults, or high-risk populations.
Preferred Qualifications:
· Associate's or bachelor's degree in public health, health education, social work, nursing, gerontology, nutrition, kinesiology, or a related field.
· Familiarity with local healthcare providers, senior services, social service agencies, and other community resources in southeastern Indiana.
