Full Job Description

Jefferson Manor is looking for experienced MDS Coordinators to help us accurately capitalize on the high-quality care provided by our clinical team.

QUALIFICATIONS:

Licensing and certification: Must hold a current, unencumbered license in this state as a registered nurse or licensed practical nurse.

Education: Must be a graduate of an accredited educational institution (school of nursing) or a graduate of a state approved school of practical nursing.

Experience: At least two years nursing experience preferred, with long-term care experience and especially MDS experience given precedence.

SPECIAL DEMANDS: Must possess skills to work with residents, resident’s families, and the rest of our team. Must be able to communicate and work effectively with other Clinical Care Coordinators.

JOB KNOWLEDGE:

Thorough knowledge of the operations of a long-term care facility; thorough knowledge of Federal and State regulations for assessment of long-term care residents; thorough knowledge of Medicare case-mix requirements; thorough knowledge of minimum data set requirements; good knowledge of administrative procedures; good knowledge of federal, state, local and professional policies, rules, regulations, and procedures as they relate to the nursing service of a skilled nursing facility; operate within scope of Nurse Practice Act; good knowledge of the care of long-term geriatric residents; good knowledge of automated system; familiarity with health practices and procedures; ability to coordinate and direct the activities of a multi-disciplinary staff; ability to maintain cooperative working relationships; ability to communicate effectively both orally and in writing; ability to identify problems and needs in order to formulate and implement appropriate care planning techniques; possess sound and mature judgment; objectivity; resourcefulness; tact; and thoroughness.

Pay: $26.00 - $35.00 per hour
