Compensation: $120,000 - $160,000+ Total Compensation (Base Salary + Productivity Incentive Program) + Mileage Reimbursement

Every day you'll travel throughout the Brainerd Lakes region caring for older adults in the communities they call home.

You'll become a trusted clinical partner for residents, families, nurses, therapists, caregivers, and community leadership while helping patients avoid unnecessary hospitalizations and maintain the highest possible quality of life.

Rather than treating isolated medical conditions, you'll care for the whole patient while building lasting relationships and improving continuity of care across every setting.

What You'll Be Doing

Provide comprehensive primary care throughout senior living communities
Perform routine provider visits and comprehensive patient assessments
Diagnose and manage acute and chronic medical conditions
Develop individualized treatment plans
Coordinate care following hospitalization and rehabilitation
Collaborate with physicians, nurses, therapists, social workers, and caregivers
Educate patients and families on disease management and preventative care
Participate in specialty programs and quality improvement initiatives
Help reduce avoidable hospitalizations and emergency department visits

We're searching for a Nurse Practitioner who enjoys practicing independently while collaborating with an outstanding interdisciplinary team.

Required Qualifications

Active Minnesota Nurse Practitioner license in good standing
Strong assessment, diagnostic, and clinical decision-making skills
Ability to practice independently while collaborating with physicians
Passion for caring for older adults and medically complex patients
Reliable transportation and willingness to travel throughout the Brainerd Lakes region

Preferred Qualifications

Experience with advanced wound care
Strong chronic disease management experience
Background in senior care, post-acute care, or community-based medicine
Wound Care Certification (WCC) is highly desirable
