Full Job Description

Bend, Medford, Roseburg, or Newport, Oregon (hybrid remote). Part-time, 16-20 hours per week, with occasional regional travel.

Build Something That Matters, Across Rural Oregon

Seniors at Home is a clinical care practice helping older adults stay independent and healthy in their own homes through proactive chronic care management and remote patient monitoring. We fill the gap between “too much care” and “not enough care,” keeping seniors from falling through the cracks of the medical system.

We're expanding this model into rural communities across Oregon, and we're looking for the person who will make that expansion happen on the ground. This is a relationship-driven role for someone who loves getting out into a community, building trust, and turning connections into results. You'll be the face of Seniors at Home in new markets: opening doors with local partners, standing up the operations behind each new region, and helping rural seniors get care they didn't know was available to them.

What You Will Do

Develop partner relationships in new markets. Identify and build relationships with the organizations that connect seniors to care in each rural community: Area Agencies on Aging, senior centers, Medicare counseling programs, rural health clinics, hospital care-management and discharge teams, tribal elder services, and senior-provider networking groups. You'll introduce our services, earn referral relationships, and keep those relationships warm and productive over time. This is the heart of the job.

Represent Seniors at Home publicly. Give presentations to partner staff, community groups, and at senior resource fairs and events. Speak confidently from the stage and one-on-one. Attend the networking rooms and advisory councils where the regional aging network gathers, and become a familiar, trusted presence in each market.

Drive awareness through events. Plan our presence at senior expos, resource fairs, and community events across our regions: booking booths and sponsorships, staffing tables, and creating opportunities to reach seniors and referral partners directly.

Enroll rural patients. Talk with prospective patients and their families, explain how our care works, and help them get started. There's a real relationship-sales element here; you're often the first person who helps a senior understand that this kind of support exists and is available to them.

Stand up the operations behind each expansion. Coordinate with our vendors and partners to get each new region running: devices, logistics, scheduling, and the pieces that have to be in place before we can serve patients there. Work with our internal team to build repeatable playbooks so each new market launches faster than the last.

Own the market-entry plan. Execute our market-entry strategy region by region, track progress against clear referral and enrollment goals, and report on what's working. You'll have a documented playbook to run, and the latitude to improve it.

A confident presenter. You're polished in front of groups and from a stage, and equally good one-on-one across a kitchen table.
Self-directed. You can own a territory, manage many relationships at once, and keep things moving without close supervision.
Growth-minded. You have the relationship-sales instincts to turn interest into enrolled patients and signed referral relationships.
Mission-driven. You're genuinely drawn to serving seniors and rural communities.

Compensation

$35-$41 / hour, commensurate with experience and skills.
Mileage reimbursed at the IRS standard rate for work-related travel.

Qualifications

Proven experience in community outreach, partnership development, business development, sales, or a closely related field (required).
Demonstrated comfort and skill presenting to groups and speaking publicly (required).
Based in or near one of our Oregon hub communities (Bend, Medford, Roseburg, or Newport), with a reliable vehicle and willingness to travel occasionally across Central, Southern, and Coastal Oregon (required).
Background or familiarity with seniors, healthcare, home health, hospice, Medicare, senior services, or aging-network organizations is strongly preferred.
Excellent communication and relationship skills, dependable, and able to work independently.
This is not a clinical role. No nursing or medical license is required, however can be helpful if you have this experience.

Position Details

Hybrid remote, based in one of our Oregon hub communities (Bend, Medford, Roseburg, or Newport). Relationship management and planning can be done from home; the role includes occasional in-person travel to partner organizations, events, and patients across our expansion regions (especially in the early market entry phase).
Part-time, about 16-20 hours per week, with a flexible schedule that will include some events.
Compensation and mileage as described above.

Pay: $37.00 - $41.00 per hour
