Full Job Description

Job Overview
Join our compassionate team as an In Home Caregiver and make a meaningful difference in the life of an individual requiring personalized care and support. In this vital role, you will provide comprehensive assistance with daily living activities, ensure safety through attentive patient observation, and promote independence within the comfort of the client's home. Your dedication will foster a nurturing environment, helping our client maintain their dignity and quality of life. This position offers an engaging opportunity to deliver exceptional home health care while building a rewarding relationship with the one you serve.

Responsibilities

Assist client with activities of daily living (ADLs), including bathing, dressing, grooming, and personal hygiene.
Monitor patient health by checking vital signs, observing symptoms, and reporting/recording changes to healthcare professionals.
Serve prepared nutritious meals tailored to individual dietary needs and assist with feeding by g-tube, training provided.
Support mobility and transfers using proper techniques with ceiling lifts and slings
or ramps for patient transfers which ensure safety.
Administer medications accurately according to care plans while maintaining compliance with HIPAA regulations.
Maintain detailed documentation of care provided, patient progress, and any incidents or observations.
Provide transportation assistance for medical appointments, errands, or community activities to promote social engagement.
Perform light cleaning tasks to uphold a safe and hygienic environment within the home setting.
Communicate effectively with clients’ families and healthcare team to ensure coordinated care and support.

Experience

Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred.
Demonstrated ability in patient observation, monitoring vital signs, and recognizing changes in health status.
Hands-on experience with medication administration, first aid, CPR certification, and manual handling techniques such as heavy lifting or patient transfers.
Familiarity with developing and following individualized care plans tailored to client's unique needs.
Experience working with individuals with developmental disabilities or neurodevelopmental delays.
Strong communication skills for effective client interaction and collaboration with healthcare providers.
Ability to perform light housekeeping tasks while maintaining a focus on client safety and comfort.
