Full Job Description

JOB SUMMARY

An important member of the interdisciplinary team (IDT), the Hospice Aide provides hands-on personal care to hospice patients during a vulnerable and sensitive time. The Hospice Aide plays a vital role in ensuring patient comfort, dignity, and safety while reflecting HSK’s core values of being United, Radically Available, Honor-driven, Exceptional, and Compassionate in every interaction.

The patient and family place significant trust in the Hospice Aide. One of the primary goals of this role is to promote cleanliness, comfort, and dignity while supporting quality of life at end of life. The Hospice Aide works under the direction of the RN and contributes to coordinated, team-based care.

ROLE

The Hospice Aide provides direct personal care services as assigned and directed by the RN, contributing to a United interdisciplinary approach to patient care.

Care is delivered with Compassionate attention to the physical and emotional needs of the patient, ensuring dignity, respect, and comfort are maintained at all times. The Hospice Aide acts with Honor by maintaining confidentiality, respecting patient privacy, and adhering to professional boundaries.

The Hospice Aide demonstrates Radically Available responsiveness to patient needs within the scope of assignment and communicates changes in condition promptly to the RN. All care is delivered in alignment with Exceptional standards of safety, cleanliness, and quality hospice support.

RESPONSIBILITIES / DUTIES (but not limited to)

Provide personal care including bathing in shower, tub, or basin, demonstrating Compassionate care that preserves dignity and comfort.
Shave patient upon request, maintaining Honor-driven respect for patient preferences.
Provide skin care including lotions, barrier creams, and simple makeup application to support comfort and self-esteem.
Perform hair care including shampooing, drying, combing, brushing, and simple styling with Compassionate attention to patient comfort.
Provide oral care including brushing teeth, denture care, and mouth care to ensure Exceptional hygiene and comfort.
Provide nail care, including cleaning and polish per patient request, respecting patient dignity and choice.
Assist with dressing patient per patient or caregiver preference, supporting a Compassionate and respectful experience.
Perform ROM exercises and ambulation assistance as directed by RN, ensuring a United care approach.
Turn and position patients to maintain safety, comfort, and skin integrity, demonstrating Exceptional attention to care standards.
Assist with feeding patients as needed, providing Compassionate support during meals.
Change bed linens and maintain a clean, safe environment.
Straighten and organize patient rooms to promote dignity and comfort.
Document care provided accurately and timely, reflecting Honor-driven accountability.
Maintain confidentiality of all patient and family information at all times (Honor).
Attend IDT meetings when possible to support a United team approach to care planning.
Maintain a safe environment and report changes or concerns promptly to the RN, demonstrating Radically Available communication and responsiveness.
Perform other duties as assigned.

Responsibilities DO NOT include: administering medications, providing spiritual or psychosocial counseling, participating in family decision-making, or sharing personal issues with patients or families.

FACILITY SUPPORT DUTIES

Remove and dispose of soiled linens appropriately, maintaining an Exceptional standard of cleanliness.
Remove trash from patient rooms and common areas.
Assist in cleaning and straightening family kitchen areas as needed.
Restock linens and supplies in patient rooms to support Exceptional care environment readiness.
Deliver meal trays as needed with Compassionate service and attention to patient comfort.

RESULTS

Patients will experience comfort, dignity, and cleanliness in their care.
Patients will remain safe and supported in their care environment.
Families and caregivers will receive meaningful assistance and support.
The facility will be maintained in a clean, organized, and respectful condition that reflects Exceptional standards of care.

REQUIREMENTS (of position)

CNA training required
Strong time management and organizational skills
Excellent interpersonal and communication skills reflecting Compassionate care
Ability to work effectively as part of a United interdisciplinary team
Ability to respond with urgency and reliability (Radically Available)
Commitment to patient dignity, safety, and ethical care practices (Honor-driven)
Ability to consistently meet high standards of care delivery (Exceptional)

RELATIONSHIPS

The Hospice Aide is a vital member of the interdisciplinary team and works under the direction of the RN. This role requires close collaboration with nursing staff to ensure coordinated, United patient care.

Because patients are often in vulnerable and intimate care situations, the Hospice Aide must demonstrate Compassionate presence, tenderness, and respect to reduce fear and promote trust. This trust strengthens the relationship between the patient, family, Hospice Aide, and RN.

The Hospice Aide is expected to communicate promptly with the RN regarding any changes in patient condition or decline in functional status. These observations are essential to supporting Exceptional clinical decision-making and may contribute to determining ongoing hospice eligibility.

PHYSICAL DEMANDS AND WORKING CONDITIONS

Requires sitting for long periods of time; occasional bending, stretching or lifting, and full range of body motion. This position requires the ability to work under high stress conditions for long periods of time. Requires corrected vision, ability to hear normal range of voice, drive, and negotiate steps, walkways or housing not designed for handicap access. May require contact with individuals with communicable disease and body fluids. Occasionally, requires maneuvering items weighing over 100 pounds. Able to work in normal office environment, use office equipment, and prepare written reports.
