Full Job Description

SUMMARY

To assist in the daily implementation of person-centered goals; to assist adults with disabilities to maximize their independence

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

· Collaborate with Development Specialists to implement goals as outlined in the Day Habilitation Service Plan (DHSP)

· Serve as a liaison for new member referrals/intakes

· Support members in their personal care, including use of a Hoyer lift.

· Support members having difficulty maintaining control

· Teach individuals to perform tasks necessary to function as independently as possible.

· Transport individuals to and from community activities.

· Participate in on-going assessment, modification, and development of program and services.

· Act as a role model, meeting expectations of the community for social norm and dress.

· Complete required trainings and maintain certifications.

QUALIFICATIONS To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

EDUCATION and/or EXPERIENCE

High School Diploma or equivalent or demonstrated ability to read and understand written directions required.

Minimum of at least one year of work experience required in a mental health setting or program for people with developmental disabilities.

CERTIFICATES, LICENSES, REGISTRATIONS

Basic First Aid and Adult CPR

Valid driver’s license and reliable vehicle
