Position Overview:

As a Pulmonologist, you will provide specialized respiratory and pulmonary care to patients across assisted living settings throughout Queens, Long Island, Bronx, and Brooklyn. This role focuses on delivering high-quality, patient-centered care while collaborating closely with interdisciplinary teams.

The ideal candidate is detail-oriented, proactive, and passionate about improving respiratory health and reducing acute pulmonary events in geriatric and post-acute populations.

Duties & Responsibilities:

Provide on-site pulmonary evaluations, diagnosis, and management for patients in assisted living, skilled nursing, and long-term care facilities
Assess and manage common pulmonary conditions such as COPD, asthma, pneumonia, chronic bronchitis, interstitial lung disease, sleep apnea, and respiratory infections
Monitor and manage patients with chronic respiratory conditions, including oxygen therapy, inhaler regimens, and medication adjustments
Interpret diagnostic information such as pulmonary function tests (PFTs), chest X-rays, CT scans, oxygen saturation trends, and relevant lab results to guide treatment plans
Prescribe and manage pulmonary medications, including bronchodilators, corticosteroids, and antibiotics, ensuring safe and effective treatment while monitoring for side effects and interactions
Collaborate with interdisciplinary care teams, including physicians, nurse practitioners, nursing staff, respiratory therapists, and facility leadership, to develop and implement individualized care plans
Provide early intervention to prevent hospitalizations and reduce respiratory complications such as exacerbations, infections, and respiratory failure
Educate patients, families, and facility staff on pulmonary conditions, disease progression, smoking cessation, prevention strategies, and treatment adherence
Maintain accurate, timely, and compliant medical documentation in accordance with organizational and regulatory standards
Ensure compliance with all state, federal, and facility-specific healthcare regulations
Support continuity of care by coordinating follow-ups, referrals (including sleep studies or advanced pulmonary care), and ongoing treatment plans
