Full Job Description

The Medical Benefits Assistance Program (MBAP) inform and provides access to free health care (HIV Drugs, Primary Care, Home Care, and APIC) for New York State residents living with HIV who are uninsured, underinsured or undocumented.

Main function: The MBAP Community Outreach Specialist will be responsible for conducting targeted educational community outreach as a means to enroll PLWHIV into the availability of ADAP and ADAP Plus, HIV Home Care Program and ADAP plus Insurance Continuation Program (APIC) with an especial focus in the uninsured, underinsured and undocumented populations.

Duties to include:

Provide outreach services in the community to inform others about the DCS’ Medical Benefits Assistance Program (MBAP) in Brooklyn.
Assists program manager in facilitating/creating linkages with other service providers.
Assist program manager in maintaining client files, AIRs statistical data, and referral logs.
Meet with clients individually to establish medical benefits eligibility.
Assist consumer in submission of all necessary documentation.
Provide appropriate referrals for follow-up care.
Conduct assigned administrative duties (mail, follow-up, etc.) and other assigned job duties.
Other duties as assigned.

Minimum Qualifications: Candidate must have a minimum of a high school diploma or GED with 2 years experience; Bachelors preferred and possess good communication and oral presentation skills. Position has a flexible schedule that includes some evening/weekend work. Knowledge of the functions and resources of public social welfare agencies preferred. Must be computer literate, and have experience in working with immigrant populations, mental health, and/or persons living with HIV/AIDS. Bi-lingual in Haitian Creole required.
