Care Navigators | Buffalo, NY

Care Navigators is seeking an experienced and highly motivated Care Manager to join our growing Health Home Care Management team serving Erie and Niagara Counties.

This position is ideal for an experienced behavioral health professional who is passionate about serving individuals with complex medical, behavioral health, and social needs. The Care Manager will provide person-centered care coordination, collaborate with community partners, and support members in achieving improved health outcomes and quality of life.

Responsibilities

Provide comprehensive Health Home Care Management services to assigned members.
Complete assessments, care plans, crisis plans, and documentation in accordance with HHUNY standards.
Coordinate medical, behavioral health, housing, social service, and community resources.
Participate in case conferences and interdisciplinary collaboration.
Maintain timely and accurate documentation in Netsmart CareManager.
Support transitions of care and assist members with complex psychosocial needs.
Collaborate with hospitals, behavioral health providers, MCOs, and community agencies.
Utilize person-centered, trauma-informed, and recovery-oriented approaches.
Conduct home and community visits throughout Erie and Niagara Counties.

Minimum QualificationsEducation

Master's degree required in one of the following fields:

Social Work
Psychology
Counseling
Public Health
Human Services
Community Mental Health
Nursing
Rehabilitation
Or another related human services field

Experience

Minimum of three (3) years of experience serving individuals with Serious Mental Illness (SMI), serious Substance Use Disorders (SUD), or other behavioral health populations.
Previous Health Home Care Management, HH+, HARP, HCBS, or Behavioral Health experience strongly preferred.
Experience with crisis intervention, motivational interviewing, person-centered care planning, and community-based services preferred.
Leadership, supervisory, mentoring, or team development experience strongly preferred.
Experience working with individuals with serious mental illness (SMI), substance use disorders (SUD), homelessness, justice involvement, and medically complex populations is highly desirable.

Knowledge and Skills

Strong understanding of behavioral health systems and community resources.
Ability to manage complex cases and collaborate effectively with multidisciplinary teams.
Excellent communication, organizational, and leadership skills.
Knowledge of Medicaid, Managed Care, and Social Determinants of Health preferred.
Experience with Netsmart CareManager, HARP, HCBS, and HHUNY processes preferred.

Preferred Qualifications

Previous HHUNY experience.
Experience serving Health Home Plus (HH+) members.
Experience with HARP and HCBS services.
Behavioral Health experience.
Motivational Interviewing and crisis intervention experience.
Experience supervising, training, mentoring, or onboarding staff.
Champion, Interim Champion, or HHUNY trainer experience preferred.
Experience with HHUNY, Health Home Plus (HH+), HARP, HCBS, and New York State behavioral health programs strongly preferred.

Additional Requirements

Valid driver's license and reliable transportation.
Ability and willingness to conduct home and community visits throughout Erie and Niagara Counties.
Ability to work independently while maintaining high standards of quality and professionalism.
