Full Job Description

Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.

A healthcare facility in State College, PA, seeks a dedicated Clinical Pharmacist for a permanent position with potential temp-to-hire options. This hybrid role balances clinical and staffing responsibilities—approximately 60% clinical and 40% staffing duties—offering a unique opportunity to impact patient care directly while supporting daily pharmacy operations.

The clinical focus lies in Population Health and Ambulatory Care, particularly managing medication regimens for patients with targeted disease states such as diabetes. As the clinical program grows, there is potential for this role to become full-time clinical, making it ideal for pharmacists passionate about expanding clinical services and enhancing patient outcomes.

Key Responsibilities:

Manage and optimize medication therapies in ambulatory care settings, focusing on chronic disease management.
Collaborate with healthcare teams to develop and implement population health strategies.
Perform staffing duties, including dispensing medications and overseeing pharmacy operations.
Utilize Meditech for clinical documentation and medication management.
Engage in patient education and counseling to promote medication adherence and safety.

Qualifications and Desired Experience:

Licensed pharmacist in Pennsylvania.
Experience in ambulatory care or inpatient settings preferred.
Completion of a residency program is strongly preferred.
Strong communication and clinical decision-making skills.
Ability to work variable days and afternoon shifts, primarily 8-hour day shifts.
