Middle Alabama Area Agency on Aging is dedicated to empowering individuals to live independently and enjoy a high quality of life by providing access to essential information, assistance, and resources. The agency specializes in serving older adults, caregivers, and individuals with disabilities across multiple counties in the region.

Position Summary

The Medicaid Waiver Services (MWS) Case Manager plays a vital role in supporting elderly and disabled clients through Alabama’s Medicaid Waiver program. This position requires adherence to ethical standards and HIPAA compliance, strong communication skills, effective time management, and the ability to foster teamwork and maintain positive working relationships.

Work Environment

This is a hybrid position. Tuesdays are mandatory in-office days, and home visit work must be conducted in the field, in clients’ homes. The role offers remote flexibility for clerical and administrative tasks. The selected candidate will report directly to their supervisor and is expected to maintain regular communication and collaboration with the team—both virtually and in person.

Essential Duties and Responsibilities

Manage a caseload of up to 40 clients and oversee service delivery by fostering positive relationships with clients, caregivers, doctors, and direct service providers (DSPs).
Complete 90% of client home visits by the 15th of each month.
Conduct monthly face-to-face visits in clients’ homes and hospitals (if admitted).
Document service delivery of the MWS Elderly and Disabled Waiver in real-time using designated software.
Arrange services through contractors and serve as a liaison with other social service agencies.
Comply with all policies and procedures of Alabama Medicaid.
Monitor case management plans, complete assessments, and draft SMART goals.
Evaluate waiver re-determinations and respond to Focus Notes for clients in the caseload.

Knowledge, Skills, and Abilities (can be acquired on the job)

Ability to establish and maintain effective working relationships with co-workers and clients.
Knowledge of social work principles and interviewing techniques.
Proficiency in Microsoft Office and general office procedures.
Strong verbal and written communication skills.
Solid understanding of English grammar, spelling, and punctuation.

Education / Experience

1. Bachelor’s degree in social work, psychology, or a related field is required.

2. Preferred experience in social work, especially with the geriatric population.

3. One year of home visit experience and/or LBSW (can be obtained through work experience).

Ability to drive to various locations as required by the position.
Ability to occasionally lift and/or move up to 10 pounds.
