Full Job Description

Job Title: Help Me Grow Care Coordinator

Location: Must be physically located in the service area (Lee, Collier, Hendry or Glades County), must have the ability to work in the office and remotely, and attend in-person and online meetings as required

Employment Type: Full-time, non-exempt

Starting Pay Rate: $20 – 23 per hour – based on experience

Reports to: Program Manager

This is a contract‑funded position subject to annual funding availability.

Healthy Start Coalition of Southwest Florida, Inc. is a nonprofit 501(c)(3) dedicated to reducing infant deaths and premature births as well as promoting healthy outcomes for babies and young children. We have served families in Collier, Glades, Hendry and Lee counties since 1992. The Healthy Start Coalition of Southwest Florida system of care includes Connect, Healthy Start, CAPTA, Nurse Family Partnership, Help Me Grow, FIMR, TEAM Dad, and the Southwest Florida Regional Diaper Bank programs. Summary

Help Me Grow is a unique, comprehensive, and integrated statewide system designed to address the need for early identification of developmental and/or behavioral concerns, and link children ages 0-8 and their families to community-based developmental and behavioral services and supports. Help Me Grow addresses early detection and intervention by building collaboration among community programs, providing a centralized access point for information and referral, promoting surveillance and screenings, as well as educating and informing the community. Help Me Grow supports families, child healthcare, early care and education, and human service providers.

The Help Me Grow Care Coordinator is responsible for assisting parents/caregivers in connecting with community services, providing ongoing care coordination and follow-up, and educating parents on developmental milestones.

Essential Duties and Responsibilities include the following:

· Respond to calls from parents, childcare providers, educators, healthcare professionals, and social service agencies about child development issues and services.

· Plan and administer screenings to children using the ASQ-3, ASQ: SE-2, M-CHAT-R/F, and the PSC.

· Assess and connect clients with services through referrals; advocate as needed; provide care coordination and follow-up.

· Develop in-depth knowledge of Help Me Grow and local systems and services focused on early childhood development, disabilities, and special healthcare needs, as well as the eligibility, referral, and application processes for these services.

· Develop and coordinate collaborative relationships with appropriate organizations and individuals.

· Participate in Help Me Grow meetings, community outreach events, and physician outreach.

· Participate in training and technical assistance related to Help Me Grow work including, but not limited to, intake, screening, referrals, and outreach.

· Document relevant information and complete data entry in System for Tracking Access to Referrals (STAR).

· Maintain program supplies, inventory, orders, mailings, and record keeping.

· Maintain participant confidentiality and integrity in keeping with Coalition policies, practices, professional standards, and program requirements.

· Perform other duties as assigned.

Experience & Skills

· At least three years of relevant experience in social work, human services, education, early childhood development, case management, or a related field, and one year of experience in community outreach and/or working with at-risk families is preferred.

· Bi-lingual in Spanish preferred.

· Ability to prioritize work requirements, work with diverse populations, and work evenings and weekends as required.

· Knowledge of services, supports, and systems that serve children and families, particularly Florida’s system for early childhood services.

· Ability to use reflective listening and motivational interviewing techniques.

· Ability to communicate effectively verbally and write in a clear, concise, and effective manner.

· Ability to work effectively with people of diverse backgrounds and demonstrate cultural sensitivity.

· Knowledge of and respect for confidentiality involving clients, stakeholders, and fellow employees.

· Ability to establish and maintain effective working relationships with others.

· Ability to work independently and collaboratively with limited supervision.

· Ability to plan, organize, and coordinate multiple work assignments and events with an attention to detail.

· Ability to solve problems and meet deadlines in a fast-paced environment.

· Skilled in software applications such as Microsoft Office for word processing, spreadsheets, and databases.

Employment Type

This is a full-time, non-exempt position. Standard hours of operation are M-F 8:30am-5:00pm. The schedule is flexible and will require some evening and weekend calls and activities. Travel within Lee, Collier, Hendry, and Glades counties is required. Benefits include Paid Time Off, Holidays, Insurance and Retirement Plan.
